MRI参数与三阴性乳腺癌组织病理学分级及淋巴结转移的相关性研究

Study on the correlation between MRI parameters and pathological grade and lymph node metastasis in triple-negative breast cancer

目的:探讨三阴性乳腺癌(TNBC)的MRI表现、TIC类型、ADC值与肿瘤大小、组织病理学分级、淋巴结转移之间的相关性。方法:收集经病理证实的TNBC患者102例,均行乳腺MRI平扫、DWI和动态增强扫描。比较TNBC不同肿瘤大小、组织病理学分级、有无淋巴结转移的MRI表现和参数。肿瘤大小、组织病理学分级、淋巴结转移与TIC类型及ADC值的相关性行Spearman相关分析。结果:不同肿瘤大小(T1、T2、T3)、不同组织病理学分级(Ⅰ~Ⅱ级、Ⅲ级)、有无淋巴结转移患者的肿块形态、边缘、强化特征、T2WI信号差异均无统计学意义(均P> 0.05)。组织病理学分级、淋巴结转移与TIC分型及ADC值均有相关性(均P<0.05)。结论:TIC分型越高或ADC值越低,TNBC的组织病理学分级越高,淋巴结转移的风险越大。

Objective:To investigate the correlation between MRI performance,TIC,ADC value and tumor size,histopathological grades,lymph node metastasis in patients with triple-negative breast cancer(TNBC).Methods:A total of 102 patients with TNBC who underwent MRI and then confirmed with pathology in our hospital were chosen to study.All patients underwent breast MRI plain scan,DWI,and dynamic contrast-enhanced MRI.MRI features and parameters were compared between TNBC patients with different tumor sizes,histopathological grades,or with or without lymph node metastases,and Spearman analysis was used for correlation.Results:There were no significant differences between TNBC patients with different tumor sizes(T1,T2,T3),histopathological grades(grades Ⅰ ~ Ⅱ,Ⅲ ),and with or without lymph node metastases on the lesion morphology,margins,enhancement characteristics,T2 WI signal(all P> 0.05).The histopathological grade,and lymph node metastasis were correlated with TIC or ADC value.In particular,the correlations between histopathological grade,lymph node metastasis and TIC and ADC were statistically significant(all P<0.05).Conclusions:For TNBC patients,the higher the TIC classification or the lower the ADC value,the histopathological grades may be higher,the risk of lymph node metastasis may be greater.
