摘要
目的筛选非心脏手术后心肌损伤的危险因素。方法回顾性收集2017年6月至2018年6月行非心脏手术后进入北京大学第一医院外科重症医学科的成年患者病历资料。以术后3 d内出现血清cTnI浓度>0.06 μg/L作为心肌损伤的诊断标准,将患者分为心肌损伤组和非心肌损伤组。通过查询电子病历系统中的资料获得患者的围术期资料,将单因素方差分析差异有统计学意义的变量进行logistic回归分析,筛选非心脏手术后心肌损伤的危险因素。结果共纳入752例患者,非心脏手术后心肌损伤的发生率为15.7%。急诊手术及术中乳酸浓度最高值是非心脏手术后心肌损伤发生的独立危险因素。结论急诊手术及术中乳酸浓度最高值是非心脏手术后心肌损伤发生的独立危险因素。
Objective To identify the risk factors for myocardial injury after non-cardiac surgery.Methods The medical records of adult patients admitted to the intensive care unit of Peking University First Hospital after non-cardiac surgery from June 2017 to June 2018 were retrospectively collected.Myocardial injury was defined as serum cardiac troponin I concentration>0.06μg/L within 3 days after operation,and the patients were divided into myocardial injury group and non-myocardial injury group.The perioperative data of patients were obtained through searching the electronic medical record system.The variables of which P values were less than 0.05 would enter the logistic regression analysis to stratify the risk factors for myocardial injury after non-cardiac surgery.Results A total of 752 patients were included in the study,and the incidence of myocardial injury occurred after non-cardiac surgery was 15.7%.Emergency operation and highest lactic acid concentration during operation were the independent risk factors for myocardial injury after non-cardiac surgery.Conclusion Emergency operation and highest lactic acid concentration during operation are the independent risk factors for myocardial injury after non-cardiac surgery.
