经尿道前列腺切除术后膀胱冲洗堵管的影响因素分析

Influencing Factors Induced Blockage of Bladder Irrigation after Transurethral Resection of Prostate

目的探索引起经尿道前列腺切除术(transurethral resection of prostate,TURP)后患者持续膀胱冲洗堵管的影响因素,为防范术后膀胱冲洗堵管提供参考。方法便利抽样法选取2016年11月至2018年12月上海市某三级甲等综合性医院泌尿外科194名TURP术后持续膀胱冲洗患者为研究对象,采用自行设计的TURP术后持续膀胱冲洗堵管问卷对其进行调查,并分析TURP术后患者持续膀胱冲洗堵管的影响因素。结果TURP术后持续膀胱冲洗共发生堵管34例(17.53%)。单因素分析显示,是否有糖尿病病史、不同膀胱冲洗速度、术前凝血酶原时间以及术中出血量在TURP术后持续膀胱冲洗堵管方面,差异均有统计学意义(均P<0.05)。多因素分析显示,糖尿病病史、膀胱冲洗速度、术前凝血酶原时间为TURP术后持续膀胱冲洗堵管的主要影响因素(均P<0.001)。结论针对TURP术后膀胱冲洗堵管的影响因素,临床医护人员应制订针对性防范措施,有计划地降低堵管事件的发生。

Objective To explore the influencing factors which induced the blockage of bladder irrigation system after transurethral resection of prostate(TURP),and provide references for taking prevention interventions to reduce the blockage rate.Methods By convenience sampling method,the medical data and demographic information of 194 patients with bladder irrigation after TURP from November 2016 to December 2018 were selected and analyzed retrospectively.The self-designed questionnaire of the blockage of bladder irrigation after TURP was used to investigate and analyze the influencing factors of the blockage of bladder irrigation system after TURP.Results Totally 34 cases of continuous bladder irrigation after TURP were blocked(17.53%).The univariate analysis showed that a history of diabetes mellitus,different bladder flushing speed,preoperative prothrombin time and intraoperative bleeding volume were related to continuous bladder flushing and tube plugging after TURP,the differences were statistically significant(all P<0.05).Multivariate analysis showed that diabetes history,bladder washing speed and prothrombin time before TURP were the main influencing factors of blockage of bladder irrigation after TURP(all P<0.001).Conclusions Based on the influencing factors of the blockage of bladder irrigation after TURP,the clinical medical staff should formulate targeted preventive measures to reduce the occurrence of tube blockage events.
