妊娠期肝内胆汁淤积症胆汁酸、肝酶指标对围生儿预后的影响 被引量:2

摘要 目的研究妊娠期肝内胆汁淤积症(ICP)胆汁酸、肝酶指标对围生儿预后的影响。方法将2012.06~2017.06间于我院进行常规产检并住院分娩的131例ICP患者与同期150例正常产妇纳为研究对象,根据ICP患者血清胆汁酸(TBA)水平将其分为轻度组(TBA<40μmol/L,n=79)与重度组(TBA≥40μmol/L,n=52),并将150例正常产妇纳为对照组,评估胆汁酸与肝酶指标对ICP围生儿预后的影响。结果三组TBA、ALT、AST及TBIL水平存在显著性差异(P>0.05);三组胎儿宫内窘迫、早产、羊水污染、Apgar评分<7分、新生儿吸入性肺炎等围生儿不良预后事件发生率存在显著性差异(P<0.05);单因素分析提示,ICP产妇年龄≥35周岁、发病时间<34周、TBA≥40μmol/L、ALT≥200U/L、AST≥200U/L、TBIL≥28μmol/L均是ICP围生儿不良预后不良的危险因素(P<0.05)。结论 ICP将加大围生儿不良预后发生率,产前密切监控ICP患者TBA及肝酶指标在预测及改善围生儿预后中具有良好的应用价值。
