摘要
目的探讨宫颈癌患者血清上皮型钙黏蛋白(E-cad)、干扰素γ(IFN-γ)、转化生长因子β(TGF-β)表达意义及对预后的影响.方法选取本院2015年4月至2016年4月宫颈癌患者60例(宫颈癌组)、宫颈鳞状上皮内病变(CIN)患者35例(CIN组)、宫颈炎患者42例(宫颈炎组),比较3组、不同病理特征患者血清E-cad、IFN-γ、TGF-β水平,分析各血清指标对宫颈癌的诊断价值.随访3年,比较不同血清E-cad、IFN-γ、TGF-β水平患者预后情况.结果宫颈癌组血清E-cad、TGF-β高于CIN组、宫颈炎组,IFN-γ低于CIN组、宫颈炎组(P<0.05);血清E-cad、IFN-γ、TGF-β联合诊断CIN、宫颈炎、宫颈癌AUC均>单一诊断;血清E-cad、TGF-β与临床分期、肿瘤直径、淋巴结转移呈正相关,与分化程度呈负相关,IFN-γ与临床分期、肿瘤直径、淋巴结转移呈负相关,与分化程度呈正相关(P<0.05);随访3年,E-cad、TGF-β、IFN-γ高危组、低危组生存曲线对比,差异有统计学意义(P<0.05).结论血清E-cad、IFN-γ、TGF-β水平在宫颈癌患者中呈异常表达,并与宫颈癌患者病理特征存在一定相关性,可作为诊断宫颈病变类型、评估宫颈癌患者预后的潜在有效因子.
Objective To explore the significance of serum epithelial cadherin(E-cad),interferonγ(IFN-γ),and transforming growth factorβ(TGF-β)levels in patients with cervical cancer and their impact on prognosis.Methods From April 2015 to April 2016,60 patients with cervical cancer(cervical cancer group),35 patients with cervical squamous intraepithelial lesion(CIN)(CIN group),and 42 patients with cervicitis(cervicitis group)were selected.The serum E-cad,IFN-γ,and TGF-βlevels of the three groups with different pathological characteristics were compared,and the diagnostic value of each serum index for cervical cancer was analyzed.Follow-up for three years,the prognosis of patients with different serum E-cad,IFN-γ,TGF-βlevels were compared.Results Serum E-cad and TGF-βin the cervical cancer group were higher than those in the CIN group and cervicitis group,and IFN-γwas lower than those in the CIN group and cervicitis group(P<0.05).Serum E-cad,IFN-γ,TGF-βcombined AUC for diagnosis of CIN,cervicitis,and cervical cancer AUC were greater than that of the single diagnosis.Serum E-cad and TGF-βwere positively correlated with clinical stage,tumor diameter,and lymph node metastasis,and negatively correlated with the degree of differentiation.IFN-γwas negatively correlated with clinical stage,tumor diameter,and lymph node metastasis,and were positively correlated with the degree of differentiation(P<0.05).After 3 years of follow-up,there was a statistically significant difference in the survival curves of the E-cad,TGF-β,and IFN-γhigh-risk and low-risk groups(P<0.05).Conclusion Serum levels of E-cad,IFN-γ,and TGF-βare abnormally expressed in cervical cancer patients and have a certain correlation with the pathological characteristics of cervical cancer patients.They can be used as potential effective factors for diagnosing cervical lesion types and evaluating the prognosis of cervical cancer patients.
