糖尿病足溃疡患者预后特点及危险因素分析

Prognostic Characteristics and Risk Factors of Diabetic Foot Ulcer Patients

目的对糖尿病足溃疡患者的预后特点以及对预后产生影响的危险因素进行分析研究,为临床中对糖尿病足溃疡患者的临床防治以及预后提供有效的治疗依据。方法从该院在2016年7月—2019年7月间,收治的糖尿病足溃疡患者中随机抽取60例患者,根据患者是否得到治愈进行分组,治愈组患者有41例,未治愈组患者有19例,对两组患者进行问卷调查,分析未治愈的影响因素,从而对患者的预后特点以及影响预后的因素进行分析。结果糖尿病足溃疡患者通过治疗,治愈率达到了68.33%(41/60),未治愈率为31.67%(19/60),通过多因素分析,发现影响预后的主要因素有糖尿病足溃疡患者的病程长短、糖化血红蛋白水平(HbAc)、周围神经病变情况、缺血情况、是否发生感染情况、出现溃疡的数目,糖尿病病程越长、糖化血红蛋白的水平越高、周围神经病变发生概率越高、缺血情况的程度越强、感染的程度越强、溃疡的数目越多,未治愈的概率就越高,就会对患者的预后产生严重的影响(t=6.265、6.432、6.522、5.491、5.258、5.639,P<0.05)。结论对于糖尿病足溃疡患者来说,病程越长、糖化血红蛋白的水平越高、周围神经病变发生概率越高、缺血情况的程度越强、感染的程度越强、溃疡的数目越多的患者,需要格外关注,该类患者的预后会受到较大的影响。

Objective To analyze the prognosis characteristics of diabetic foot ulcer patients and the risk factors that affect prognosis,and to provide effective treatment basis for the clinical prevention and treatment of diabetic foot ulcer patients in the clinic.Methods From July 2016 to July 2019,60 patients with diabetic foot ulcers were randomly selected from the hospital and grouped according to whether the patients were cured.There were 41 patients in the cured group and those in the uncured group of 19 cases,a questionnaire survey was performed on the two groups of patients to analyze the uncured influencing factors,so as to analyze the prognosis characteristics of the patients and the factors affecting the prognosis.Results After treatment,the cure rate of diabetic foot ulcer patients reached 68.33%(41/60) and the uncured rate was 31.67%(19/60).Through multivariate analysis,it was found that the main factors affecting the prognosis were those of diabetic foot ulcer patients.Duration of disease,level of glycated hemoglobin(HbAc),condition of peripheral neuropathy,ischemia,presence of infection,number of ulcers,longer duration of diabetes,higher level of glycated hemoglobin,higher probability of peripheral neuropathy,the stronger the blood condition deficiency,the stronger the infection,and the greater the number of ulcers,the higher the probability of uncured disease will have a serious impact on the patients prognosis(t=6.265,6.432,6.522,5.491,5.258,5.639,P<0.05).Conclusion For patients with diabetic foot ulcers,patients with longer disease duration,higher levels of glycated hemoglobin,higher probability of peripheral neuropathy,more severe ischemic conditions,more sewere infections,and more ulcers,need special attention,the prognosis of such patients will be greatly affected.
