摘要
目的探讨在基层医院开展早期乳腺癌前哨淋巴结活检的可行性。方法该文选取了该科2017年6月—2019年6月期间收治的48例早期浸润性乳腺癌患者,对这48例早期乳癌患者使用纳米炭示踪进行前哨淋巴结活检,同时常规进行腋窝淋巴结清扫术;按照手术时间的先后顺序分别为A,B组,每组24例,所有患者在进行改良根治术前先行腋窝前哨淋巴结活检,统计各组的前哨淋巴结活检成功率、假阴性率及灵敏度。结果 A组前哨淋巴结活检的成功率为95.83%,假阴性率为0.00%,灵敏度为100.00%;B组分别为70.83%,23.52%,76.74%;各组间差异有统计学意义(χ2=5.400,P=0.020)。结论在规范化地进行前哨淋巴结活检术后,可基本掌握前哨淋巴结活检术的手术技术;经过手术实践后,可以达到较为熟练的程度。采用纳米炭示踪前哨淋巴结活检方法简单、易行,适合基层医院开展。
Objective To explore the feasibility of sentinel lymph node biopsy for early breast cancer in primary hospitals.Methods This article selected 48 patients with early invasive breast cancer treated in the department from June 2017 to June 2019.Sentinel lymph node biopsy was performed using nano-carbon tracing for these 48 patients with early breast cancer,and axillary lymph nodes were routinely performed;Group A and Group B,24 patients in the order of operation time.All patients underwent axillary sentinel lymph node biopsy before modified radical operation,and statistics on the success rate,false negative rate,and sensitivity of sentinel lymph node biopsy in each group.Results The success rate of sentinel lymph node biopsy in group A was 95.83%,the false-negative rate was 0.00%,and the sensitivity was 100.00%;group B was 70.83%,23.52% and 76.74%,respectively;the differences between groups were statistically significant(χ2=5.400,P=0.020).Conclusion After the sentinel lymph node biopsy is performed in a standardized way,the surgical technique of sentinel lymph node biopsy can be basically mastered;after surgical practice,it can reach a more proficient level.Sentinel lymph node biopsy using nano-carbon is simple and easy,and it is suitable for primary hospitals.
