比较腹腔镜直肠癌手术与开腹直肠癌根治术治疗老年直肠癌患者的疗效

Comparison of the Efficacy of Laparoscopic Rectal Cancer Surgery with Open Rectal Cancer Radical Surgery in Elderly Patients with Rectal Cancer

目的比较腹腔镜直肠癌手术与开腹直肠癌根治术治疗老年直肠癌患者的疗效。方法选取该院在2017年1月—2019年8月接收的70例老年直肠癌患者作为该次的研究对象,分成两组进行对比分析。观察两组患者对不同手术方案的评价,手术情况比较,并发症发生率比较。结果通过观察组手术治疗方式,患者的总满意度为97.14%,通过对照组手术治疗方案,患者的总满意度为77.14%,可以看出,观察组手术治疗方案的总满意度要明显高于对照组(χ2=4.590,P=0.032);采用不同的手术治疗方式,其手术时间、淋巴结清数量、住院总时间差异无统计学意义(P>0.05)。观察组的术后出血量要明显比对照组少,术后通气时间也比对照组短,切口长度也比对照组短(P<0.05);采用观察组手术方案,患者出现1例肠梗阻、1例切口感染、1例肠漏,共计3人,并发症总比例为8.57%;采用对照组手术方案,患者出现2例肺部感染、3例肠梗阻、3例切口感染、3例肠漏,共计11人,并发症总比例为31.42%,由此可以看出,观察组并发症比率明显比对照组要低(χ2=4.375,P=0.036<0.05)。结论对老年直肠癌患者采用腹腔镜直肠癌手术,效果十分显著,并发症较低。

Objective To compare the efficacy of laparoscopic rectal cancer surgery with open rectal cancer radical surgery in elderly patients with rectal cancer.Methods A total of 70 eldery patients with rectal cancer who were accepted by the hospital from January2017 to August 2019 were selected as the subjects of this study and divided into two groups for comparative analysis.Observe the evaluation of different surgical schemes in two groups of patients,compare the surgical situation,and compare the incidence of complications.Results The total satisfaction of the patients in the observation group was 97.14%,and the total satisfaction of the patients in the control group was 77.14%.It can be seen that the total satisfaction of the surgical group in the observation group was significantly higher than the control group(χ2=4.590,P=0.032);there were no significant differences in surgical time,number of lymph node dissections,and total length of hospital stay with different surgical treatments(P>0.05).The amount of postoperative bleeding in the observation group was significantly less than that in the control group.The postoperative ventilation time was shorter than that in the control group,and the length of the incision was shorter than that in the control group(P<0.05).With the observation group’s surgical plan,1 patient had intestinal obstruction,1 incision infection,1 case of intestinal leakage,a total of 3 people,with a total complication rate of 8.57%.In the control group,the patient developed 2 cases of pulmonary infection,3 cases of intestinal obstruction,3 cases of incision infection,and 3 cases of intestinal leakage.The total complication rate was 31.42%.It can be seen that the complication rate in the observation group was significantly lower than that in the control group(χ2=1.375,P=0.036 <0.05).Conclusion Laparoscopic rectal cancer surgery is effective for elderly patients with rectal cancer,and the complications are low.
