胃癌根治术全麻苏醒期血气监测对术后并发症的预防作用分析

Analysis of the Preventive Effect of Blood Gas Monitoring During the Postoperative General Anesthesia on the Recovery of Gastric Cancer on Postoperative Complications

目的分析在胃癌根治术后麻醉的恢复期中实施血气监测对预防术后并发症的作用。方法选取2015年2月—2019年8月该院收治的54例胃癌根治术患者,按照随机数字表法分为对照组和观察组,每组各27例。对照组在全麻苏醒期实施血气监测分析,在此基础上观察组再实施血气监测指导治疗,观察两组患者的常规监测指标、血气值以及在拔管后的24 h之内发生的并发症情况。结果观察组的拔管时间(T1:6.45±4.45;T2:16.45±6.05)min明显短于对照组(T1:9.75±5.05;T2:24.05±6.55)min(t=2.548、4.429,P=0.014、0.000);观察组的PaO2、PaCO2、BE、A-aDO2、SpO2水平与对照组比较(t=4.764、5.127、6.201,P=0.000;χ2=5.333、8.882,P=0.021、0.003);观察组的术后并发症发生率为22.22%,明显低于对照组的48.15%(χ2=3.979,P=0.046)。结论在胃癌根治术后麻醉的恢复期中吸入5 min空气后,血气监测显示多项指标出现异常,临床根据结果进行指导治疗,对预防术后并发症具有重要意义。

Objective To analyze the effect of blood gas monitoring on the prevention of postoperative complications during the recovery period of anesthesia after gastric cancer radical operation.Methods Fifty-four patients with radical gastric cancer who were admitted to the hospital from February 2015 to August 2019 were divided into control group and observation group according to the random number table method,with 27 cases in each group.The control group peformed blood gas monitoring analysis during the general anesthesia recovery period,and on the basis of this,the observation group implemented blood gas monitoring and guided treatment,and observed the routine monitoring indicators,blood gas values,and complications of the patients within 24 hours after extubation.Results The extubation time of the observation group(T1:6.45±4.45;T2:16.45±6.05) min was significantly shorter than that of the control group(T1:9.75±5.05;T2:24.05±6.55)min(t=2.548,4.429,P=0.014,0.000);PaO2,PaCO2,BE,A-aDO2,and SpO2 levels in the observation group were compared with the control group(t=4.764,5.127,6.201,P=0.000;χ2=5.333,8.882,P=0.021,0.003);the postoperative complication rate in the observation group was 22.22%,which was significantly lower than the control group’s 48.15%(χ2=3.979,P=0.046).Conclusion After inhaling 5 min of air during the recovery period of anesthesia for gastric cancer,blood gas monitoring showed abnormalities in a number of indicators,and clinical treatment based on the results was of great significance in preventing postoperative complications.
