衰弱综合征与老年心力衰竭预后相关性的Meta分析

Meta analysis of the prognostic impact of frailty syndrome in elderly patients with heart failure

目的系统评价衰弱综合征对老年心力衰竭(心衰)病人预后的影响。方法检索Pubmed、Embase、Cochrane、中国期刊全文数据库、维普数据库、万方数据库,筛选文献、提取资料后,评价纳入文献的质量,并采用Stata软件进行Meta分析,评价衰弱综合征与老年心衰病人1年内再入院率、全因死亡的关系。结果总共纳入2944例病人,Meta分析纳入6项研究评价全因死亡风险,结果显示与非衰弱病人相比,衰弱增加老年心衰病人全因死亡风险(HR=1.712,95%CI:1.405~2.085)。纳入5项研究评价再入院风险,结果显示,与非衰弱病人相比,衰弱增加老年心衰病人1年内再入院风险(HR=1.751,95%CI:1.451~2.114)。结论衰弱综合征可作为评价老年心衰病人不良预后的指标。

Objective To investigate the impact of frailty syndrome on the prognosis of the elderly patients with heart failure by Meta analysis.Methods Pubmed Database,Embase Database,Cochrane Library,CNKI Database,VIP Database,Wanfang Database were searched.The relation of frailty syndrome with the prognosis of the elderly patients with heart failure were analyzed by Meta analysis.The data was extracted independently by two reviewers.The methodological quality of the trails was assessed by recommended evaluation standard.Statistical analysis was performed with STATA 14.Results A total of 2944 cases were included.The frailty syndrome was significantly related to increased all-cause mortality(HR=1.712,95%CI:1.405-4.02)and increased readmission(HR=1.751,95%CI:1.451-2.114).Conclusions Frailty syndrome may be used as a parameter for the poor prognosis of the elderly patients with heart failure.
