不同髓内钉系统治疗老年股骨粗隆间骨折的近期效果比较

Comparison on short-term effects of different intramedullary nail systems in treatment of senile femoral intertrochanteric fracture

目的探讨不同髓内钉系统在老年股骨粗隆间骨折中的应用。方法选择我院2017年2月~2019年2月收治的80例股骨粗隆间骨折患者,按照随机数字表法分为股骨近端防旋髓内钉(PFNA)组和股骨近端髓内钉(PFN)组,比较两组手术指标、髋关节功能、生活质量及并发症情况。结果PFNA组手术时间短于PFN组,术中出血量少于PFN组,差异有统计学意义(P<0.05);PFNA组髋关节功能恢复优良率高于PFN组,差异有统计学意义(P<0.05);PFNA组术后日常生活质量量表(SF-36)中各维度评分高于PFN组,差异有统计学意义(P<0.05);PFNA组术后并发症总发生率低于PFN组,差异有统计学意义(P<0.05)。结论与PFN手术相比,PFNA手术治疗股骨粗隆间骨折的效果较好,创伤较小,并发症少,有利于术后早期康复。

Objective To investigate the application of different intramedullary nail systems in senile femoral intertrochanteric fracture.Methods A tatal of 80 patients with femoral intertrochanteric fracture who admitted in the hospital from February 2017 to February 2019 were divided into the proximal anti-rotation intramedullary nail(PFNA)group and the proximal femoral intramedullary nail(PFN)group according to the random number table method,with 40 cases in each group.PFEA group accepted proximal femoral nail anti-rotation(PFNA)surgery,and PFE group accepted proximal femoral nail(PFN)surgery.The surgical indexes,hip joint function,quality of life and complications were compared between two groups.Results The operation time of PFEA group was shorter than that of PFE group,and the intraoperative bleeding volume in PFEA group was less than that in PFE group(P<0.05).The excellent rate of hip joint recovery in PFEA group was higher than that in PFE group(P<0.05).After surgery,the scores of Short Form 36-item Health Survey(SF-36)scale in PFEA group were higher than those in PFE group(P<0.05).The total incidence of postoperative complications in PFEA group was lower than that in PFE group(P<0.05).Conclusion PFNA surgery is effective in treating femoral intertrochanteric fractures,with less trauma and fewer complications,which is conducive to early postoperative rehabilitation.
