血清胃蛋白酶原Ⅰ、Ⅱ及其比值联合幽门螺杆菌抗体诊断老年早期胃癌的临床意义

Clinical significance of serum pepsinogenⅠ,Ⅱand their ratio combined with Helicobacter pylori antibody in the diagnosis of early gastric cancer in the elderly

目的探讨血清胃蛋白酶原Ⅰ、Ⅱ及其比值联合幽门螺杆菌(Hp)抗体诊断老年早期胃癌的临床意义。方法选取2018年8月~2019年7月我院收治的56例老年早期胃癌患者和胃炎患者作为研究对象,将其分为胃癌组和胃炎组,每组各28例。选取同期体检的28例健康者作为健康体检组,比较三组受试者血清胃蛋白酶原(PG)Ⅰ、Ⅱ水平及Hp抗体表达水平。结果检查显示,胃癌组的血清PGⅠ和血清PGⅡ水平低于健康体检组和胃炎组,胃炎组的血清PGⅠ和PGⅡ水平低于健康体检组,差异均有统计学意义(P<0.05)。三组的血清PGⅠ/PGⅡ比较,差异无统计学意义(P>0.05)。胃癌组的Hp抗体阳性率高于胃炎组和健康体检组,胃炎组的Hp抗体阳性率高于健康体检组,差异均有统计学意义(P<0.05)。结论血清胃蛋白酶原Ⅰ、Ⅱ及其比值联合Hp抗体在老年早期胃癌诊断中的应用效果显著,可以用于诊断老年早期胃癌,具有较高的临床应用价值。

Objective To explore the clinical significance of serum pepsinogen(PG)Ⅰ,Ⅱand their ratio combined with Helicobacter pylori(Hp)antibody in the diagnosis of early gastric cancer in the elderly.Methods From August 2018 to July 2019,56 elderly patients with early gastric cancer and gastritis treated in our hospital were selected as subjects.They were evenly divided into the gastric cancer group and gastritis group,28 cases in each group.Twenty-eight healthy subjects for physical examinations were selected as the healthy physical examination group during the same period.The levels of PGⅠand PGⅡ,Hp antibody expression level were compared between the three groups.Results Check the display,the levels of PGⅠand PGⅡin the gastric cancer group were lower than those in the healthy physical examination group and gastritis group;the levels of PGⅠand PGⅡin the gastritis group were lower than those of the healthy physical examination group,the differences were statistically significant(P<0.05).There were no significant differences in the ratio of PGⅠto PGⅡbetween the three groups(P>0.05).The positive rate of Hp antibody in the gastric cancer group were higher than those in the gastritis group and the healthy physical examination group,the positive rate of Hp antibody in the gastritis group was higher than that of the healthy physical examination group,and the differences were statistically significant(P<0.05).Conclusion Serum pepsinogenⅠ,Ⅱand their ratios combined with Hp antibody obtain remarkable effects in the diagnosis of early gastric cancer in the elderly,and can be used to diagnose early gastric cancer in the elderly,and have high clinical application value.
