废用性肌萎缩患者股内侧肌的定量超声图像特点研究 被引量:5

摘要 目的:本研究运用定量超声成像技术,将废用性肌萎缩患者的各超声参数与健康受试者进行比较,探讨废用性肌萎缩患者股内侧肌的定量超声图像特点和肌肉功能变化。方法:本研究运用定量超声成像技术,采集92例受试者(健康受试者54例,废用性肌萎缩患者38例)的下肢股内侧肌厚度、平均回声强度及能量多普勒超声参数等,将废用性肌萎缩患者的各超声参数与健康受试者进行比较,探讨废用性肌萎缩患者的定量超声图像特点和肌肉功能变化,为废用性肌萎缩的超声辅助检查提供依据。同时对全部研究对象进行双下肢等速肌力测试。结果:废用性肌萎缩患者患侧的峰力矩值与健侧相比下降,差异有显著性意义(P<0.05),废用性肌萎缩患者患侧和健侧肌肉的平均回声强度均较健康人明显提高(P<0.05),废用性肌萎缩患者健侧肌肉厚度相比患侧更厚(P<0.05)。通过分析伸膝峰力矩值和各超声参数间的相关性,研究发现肌肉厚度和能量超声参数分别与峰力矩值呈正相关(r=0.614,P=0.000;r=0.312,P=0.000),回声强度与峰力矩值呈负相关(r=-0.385,P=0.000)。结论:废用性肌萎缩患者患侧伸膝峰力矩较健侧显著下降。同时废用性肌萎缩患者双侧股四头肌内侧头的质量和肌肉募集能力均较健康人有所下降。运用超声判断肌肉萎缩情况时,平均回声强度是较为可靠的指标,肌肉厚度和能量超声参数并不能准确反映肌萎缩患者的肌肉变化。 Objective:In this study,quantitative ultrasound imaging was used to compare the ultrasonic parameters of patients with disused muscular atrophy with those of healthy subjects,and to investigate the quantitative ultrasonic image characteristics and medial vastus muscle function changes of patients with disused muscular atrophy.Method:In this study,quantitative ultrasound imaging technology was used to collect the medial femoral muscle thickness,average echo intensity,and energy Doppler ultrasound parameters of 92 subjects(54 healthy subjects and 38 disused muscular atrophy patients).The ultrasound parameters of the patients with disuse muscular dystrophy were compared with the healthy subjects,and the quantitative ultrasound image characteristics and the muscle function changes of patients with disuse muscular dystrophy were discussed to provide a basis for ultrasound-assisted examination of disuse muscular atrophy.Result:The peak torque value of the affected side of the patients with disuse muscular dystrophy decreased compared with the healthy side,with the statistically significant difference(P<0.05),the average echo of the muscles in the affected side and the healthy side of the patients with disuse muscular atrophy The intensity was significantly higher than that of the healthy people(P<0.05).The thickness of the healthy side of patients with disuse muscular dystrophy was thicker than that of the affected side and healthy subjects(P<0.05).By analyzing the correlation between the knee extension torque value and each ultrasound parameter,the study found that muscle thickness and energy ultrasound parameters were positively correlated with the peak torque value(r=0.614,P=0.000;r=0.312,P=0.000),The echo intensity is negatively correlated with the peak torque value(r=-0.385,P=0.000).Conclusion:In patients with disuse muscular dystrophy,the peak torque of the affected knee is significantly lower than that of the healthy side.At the same time,the quality and muscle recruitment capacity of the bilateral quadriceps medial head of the patients with disuse muscular atrophy were lower than those of healthy people.When using ultrasound to judge muscle atrophy,the average echo intensity is a relatively reliable indicator,and muscle thickness and energy ultrasound parameters do not accurately reflect muscle changes in patients with muscle atrophy.
