Vita In-ceram渗透氧化铝陶瓷修复对根管治疗后前牙变色患者的疗效观察

Effect of Vita In—ceram Alumina Ceramic Repair on Patients with Discolored Anterior Teeth After Root Canal Treatment

目的 探讨Vita In-ceram渗透氧化铝陶瓷修复对根管治疗后前牙变色患者的疗效.方法 选取201 7年3月~2019年5月在我院经根管治疗后前牙变色的患者作为研究对象,总共68例(87颗牙).采用单盲法将其分为对照组(34例,47颗)和观察组(34例,40颗),其中对照组采用Cergogld铸瓷制作全瓷冠修复;观察组采用Vitaln-ceram渗透氧化铝陶瓷制作全瓷冠修复.通过Crystaleye比色仪分别对修复体和天然牙进行测色,计算总色差AE值,对全瓷冠与天然牙的颜色匹配度进行牙冠颜色评分(CCS),观察两种材料修复1年后前牙美学效果评分以及修复1年后牙周组织变色及牙龈炎发生率.结果 不同变色牙组两种全瓷材料△E值比较,差异具有统计学意义(P<0.05),其中轻度变色牙<中度变色牙<重度变色牙,且Vita In-ceram渗透氧化铝陶瓷修复AE值均小于Cergogld铸瓷修复(P<0.05);两种材料修复1年后前牙美学效果评分比较,Vita In-ceram渗透氧化铝陶瓷修复体外形效果评分高于Cergogld铸瓷,差异具有统计学意义(P<0.05),整体美观及面部情况方面的效果评分比较,差异均无统计学意义(P>0.05);在不同变色牙组中,Cergogld铸瓷与Vita In-ceram渗透氧化铝陶瓷的CCS比较,差异均无统计学意义(P>0.05),在重度变色牙组中,颜色匹配度高的比例,差异有统计学意义(P<0.05);两种材料修复后牙周组织变色情况以及牙龈炎发生率比较,差异均无统计学意义(P>0.05).结论 相较于Cergogld铸瓷,Vita In-ceram渗透氧化铝陶瓷对轻度变色牙、中度变色牙及重度变色牙均有较好的修复效果,且修复后矛周组织变色情况以及牙龈炎发生率较低.

Objective To investigate the effect o f Vita In-ceram alumina ceramic repair on patients with discolored anterior teeth after root canal treatment.Methods A total o f 68 patients(87 teeth)were selected between March 2017and May 2019 in our hospital after root canal treatment.They were divided into control group(34 cases,47 cases)and observation group(34 cases,40 cases)by single blind method.The control group use Cergogld cast porcelain to make all-ceramic crowns repair;the observation gr'oup use Vita In-ceram infiltrated alumina ceramic was used to make a fiill-ceramic crown to repair.The colorimetric AE values were calculated by the Crystaleye colorimeter for the restoration and the natural adjacent teeth,and the crown color score(CCS)was calculated for the color matching degree o f the all-ceramic crown and the natural adjacent teeth.The esthetic effect scores o f the anterior teeth after 1 year o f repair and the incidence o f periodontal tissue discoloration and gingivitis after 1 year o f repair were observed.Results The difference o f AE values o f two kinds o f all-ceramic materials in different color-changing teeth group was statistically significant(P<0.05),among which mild discolored te e th〈moderately discolored teeth<severe discolored teeth,and Vita In-ceram repaired alumina ceramic repair A E value was less than Cergogld cast ceramic repair(P<0.05).The scores of esthetic effect in the two teeth were compared after 1 year o f repair,the Vita In-ceram infiltrated alumina ceramic restoration has a higher appearance rating than the Cergogld cast,the difference was statistically significant(P<0.05),the scores o f overall aesthetics and facial performance were no significant difference(P>0.05).The scores o f CCS between Cergogld cast porcelain and Vita In-ceram infiltrated alumina ceramics in different discolored teeth groups were no significant difference(P>0.05),in the severely discolored tooth group,the ratio o f high color matching degree was statistically significant(P<0.05).There was no significant difference in the periodontal tissue discoloration between the two materials and the incidence o f gingivitis(P>0.05).Conclusion Compared to Cergogld cast porcelain,Vita In-ceram infiltrated alumina ceramics have a good repair effect on mildly discolored teeth,moderately discolored teeth and severely discolored teeth,and the discoloration o f periodontal tissues and the incidence o f gingivitis are low after repair.
