闭合复位治疗小儿先天性髋关节脱位的临床价值分析

Analysis of the Clinical Value of Closed Reduction in the Treatment of Congenital Hip Dislocation in Children

目的探究闭合复位治疗小儿先天性髋关节脱位的临床价值。方法方便选取该院2017年1月—2018年12月期间治疗的120例先天性髋关节脱位患儿进行病情回顾分析。在患儿家属签署知情同意书后,以年龄大小为依据,将其分为A、B、C 3组,平均每组40例患儿。A组患儿年龄在6~15个月之间,B组患儿年龄在16~24个月之间,C组患儿年龄在25~36个月之间。医护人员在术后12个月后对3组患儿髋关节功能Mckay平分、髋关节X线Severin评分及股骨头坏死几率进行分析对比。结果A组患儿髋关节功能Mckay评分优良率为100%、髋关节X线Severin评分优良率为100%;B组患儿髋关节功能Mckay评分优良率为87.5%、髋关节X线Severin评分优良率为85.0%;C组患儿髋关节功能Mckay评分优良率为85.0%、髋关节X线Severin评分优良率为80.0%(P<0.05);A、B、C 3组患儿股骨头坏死几率分别为(2.5%)、(7.5%)、(10.0%)(P<0.05)。结论闭合复位疗法对先天性髋关节脱位患儿疗效极佳,且越早进行治疗的患儿治疗有效率越高。

Objective To explore the clinical value of closed reduction in the treatment of congenital hip dislocation in children.Methods A retrospective analysis was that convenient selection of performed on 120 children with congenital hip dislocation treated in the hospital from January 2017 to December 2018.After the family members of the children signed the informed consent,they were divided into three groups A,B,and C based on their age,with an average of 40 children in each group.The children in group A were between 6-15 months,the children in group B were between 16-24 months,and the children in group C were between 25-36 months.The medical staff analyzed and compared the Mckay bisection,hip X-ray Severin score,and the probability of femoral head necrosis in the three groups of children 12 months after surgery.Results The excellent rate of Mckay score of hip function in group A was 100%and the excellent rate of Severin score of hip X-ray was 100%;the excellent rate of Mckay score of hip function was 87.5%and excellent rate of hip X-ray Severin score in group B 85.0%;excellent rate of Mckay score of hip function in group C and 80.0%of Severin score of hip X-ray(P<0.05);femoral head necrosis rates in groups A,B,C Were(2.5%),(7.5%),(10.0%)(P<0.05).Conclusion Closed reduction therapy is very effective for children with congenital hip dislocation,and the earlier the treatment more effective the treatment.
