腹腔镜下经胆囊管汇入部微切开胆道探查的临床应用分析

Clinical Application Analysis of Laparoscopic Bile Duct Incision Through the Cystic Duct

目的探讨腹腔镜下经胆囊管汇入部微切开胆道探查的临床应用效果。方法随机选取该院于2018年1月—2019年1月期间收治20例胆总管结石患者作为该次研究对象,利用回顾性分析的方法进行分析,20例患者均使用腹腔镜下经胆囊管汇入部微切开胆道探查,并联合胆道镜取石进行治疗,评价20例患者治疗后的具体疗效以及治疗的安全性。结果20例患者在经过手术治疗后,有效率为100%,平均手术时间为(81.5±2.2)min,住院天数为(2.5±1.1)d,所有患者均为出现严重不良反应,安全性较高。治疗结束10个月后,对患者进行随访,患者饮食情况、睡眠情况等情况均良好,且未出现复发与胆道狭窄等情况。结论采用腹腔镜下经胆囊管汇入部微切开胆道探查结合胆道镜取石术为胆总管结石患者进行治疗,能够有效的提高临床治疗效果,降低疾病的复发率,安全性较高,临床效果显著,具有临床应用价值。

Objective To investigate the clinical application of laparoscopic micro-incision biliary tract exploration through the cystic duct entry.Methods Twenty patients with choledocholithiasis were convenienty selected in the hospital from January 2018 to January 2019 as the research object.The analysis was performed using retrospective analysis.All 20 patients were treated with laparoscopic transcystic duct drainage.The incision of the biliary tract was performed by microtomy and combined with choledocholithotomy for treatment,and the specific efficacy and safety of the treatment were evaluated in 20 patients.Results After the operation of 20 patients,the effective rate was 100%,the average operation time was(81.5±2.2)min,and the length of hospital stay was(2.5±1.1)d.All patients had serious adverse reactions,and the safety was relatively safer.Ten months after the end of treatment,the patients were followed up.The patients'diet and sleep were good,and no recurrence or biliary stricture occurred.Conclusion The use of laparoscopic micro-incision biliary exploration through the cystic duct infusion combined with choledocholithotomy for the treatment of patients with common bile duct stones can effectively improve the clinical treatment effect,reduce the recurrence rate of the disease,have higher safety and the clinical effect is remarkable and has clinical application value.
