食管癌调强放疗与三维适形放疗的疗效分析

Efficacy Analysis of Intensity-modulated Radiotherapy and Three-dimensional Conformal Radiotherapy for Esophageal Cancer

目的分析对比在食管癌患者的治疗中实施调强放疗和三维适形放疗的临床效果。方法方便选取2017年1月—2019年1月在该院进行治疗食管癌的患者,共50例,根据抽签盲选的形式将其分为S组和D组,D组实施三维适形放疗进行对比治疗,S组实施调强放疗进行治疗,对比两组患者的治疗效果以及出现不良反应的概率。结果S组患者临床治疗中CR6例(24.00%)、PR10例(40.00%)、SD8例(32.00%)、PD1例(4.00%),临床治疗总有效率(96.00%)优于D组患者的临床治疗效果(92.00%),两组比较差异无统计学意义(χ^2=1.190,P>0.05);S组患者出现不良反应中放射性食管炎2例(8.00%)、上消化道反应1例(4.00%)、放射性肺炎3例(12.00%)、骨髓抑制2例(8.00%),总发生率为32.00%低于D组患者出现不良反应的概率52.00%,数据比较差异有统计学意义(χ^2=4.252,P<0.05)。结论在食管癌患者的临床治疗中实施调强放疗和三维适形放疗,两者在其应用过程中均可获得比较满意的结果,但是相比较而言调强放疗治疗的总有效率有更佳的趋势,而且出现的不良反应情况也相对少些,这说明调强放疗不仅治疗效果有更佳的趋势,安全性也更高,更具有推广价值。

Objective To analyze and compare the clinical effects of IMRT and 3D conformal radiotherapy in the treatment of patients with esophageal cancer.Methods A total of 50 patients with esophageal cancer who were treated in the hospital from January 2017 to January 2019 were convenient selected.They were divided into S group and D group according to the form of blind selection by lot.Group D was subjected to three-dimensional conformal radiotherapy for comparison.In treatment group S,IMRT was performed for treatment,and the treatment effect and the probability of adverse reactions were compared between the two groups.Results In group S,6 cases of CR(24.00%),10 cases of PR(40.00%),8 cases of SD(32.00%),and 1 case of PD(4.00%),the total effective rate of clinical treatment(96.00%)was better than that of group D.Clinical treatment effect(92.00%),and the difference between the two groups was not statistically significant(χ^2=1.190,P>0.05);2 cases of radiation esophagitis(8.00%),and Gastrointestinal reactions in 1 case(4.00%),3 cases of radiation pneumonia(12.00%),and 2 cases of bone marrow suppression(8.00%),the total incidence rate was 32.00%lower than the probability of adverse reactions in group D patients was 52.00%,and the difference was statistically significant(χ^2=4.252,P<0.05).Conclusion In the treatment of esophageal cancer patients,IMRT and 3D conformal radiotherapy are performed.Both of them can obtain satisfactory results in the application process,but the overall effective rate of IMRT is better,and there are relatively few adverse reactions,which shows that IMRT not only has a better trend of treatment,but also has higher safety and more promotion value.
