脊柱外伤的CT与MRI影像诊断价值对比分析

Comparative Analysis of the Diagnostic Value of CT and MRI in Spinal Trauma

目的对比分析脊柱外伤的CT和MRI影像诊断价值。方法在2018年6月—2019年6月该时间段内,从该院收治的脊柱外伤患者中方便选取84例作为样本,并进行相关的对比工作。选取的样本全部接受CT检查及MRI检查,将手术结果作为参考依据,对CT和MRI诊断价值进行比较分析。结果CT诊断椎弓骨折及椎弓骨折线的检出率(57.34%、95.74%)明显高于MRI(37.49%、80.95%),MRI诊断脊髓、软组织及神经根损伤的检出率(46.42%、30.95%、15.47%)明显高于CT(38.09%、29.76%、10.71%),差异有统计学意义(P﹤0.05)。在脊髓损伤、软组织损伤、前纵韧带、后纵韧带、神经根损伤(χ^2=3.295、3.955、7.907、9.094、4.658,P=0.043、0.041、0.004、0.003、0.38)。结论在椎弓骨折、骨折线上方面,CT表现更好,但是MRI可以更好地精准确定脊柱外伤空间,而且可以促使由于外伤造成的脊髓病变及损伤进行明显地显现。CT与MRI存在各自的优缺点,应该结合使用,实现优势互补。

Objective To compare the diagnostic value of CT and MRI images of spinal trauma.Methods During the period from June 2018 to June 2019,84 cases of spinal trauma patients treated in the hospital were convenient selected as samples and related comparisons were performed.All the selected samples were subjected to CT and MRI examinations,and the surgical results were used as a reference basis to compare and analyze the diagnostic value of CT and MRI.Results The detection rates of vertebral arch fractures and vertebral arch fracture lines by CT(57.34%,95.74%)were significantly higher than those of MRI(37.49%,80.95%).The detection rates of MRI in the diagnosis of spinal cord,soft tissue and nerve root injuries(46.42%,30.95%,15.47%)were significantly higher than CT(38.09%,29.76%,10.71%),and the differences were statistically significant(P<0.05).For spinal cord injury,soft tissue injury,anterior longitudinal ligament,posterior longitudinal ligament,and nerve root injury(χ^2=3.295,3.955,7.907,9.094,4.658,P=0.043,0.041,0.004,0.003,0.38).Conclusion CT has better performance in vertebral arch fractures and fracture lines,but MRI can better determine the spinal trauma space,and can promote the obvious manifestation of spinal cord lesions and injuries caused by trauma.CT and MRI have their own advantages and disadvantages and should be used in combination to achieve complementary advantages.
