摘要
目的探讨脐血25-羟基维生素D水平对6个月龄婴儿神经发育的影响。方法前瞻性选取2016年12月至2017年12月期间新生儿142例,用酶联免疫吸附试验检测脐血25-羟基维生素D水平,根据脐血25-羟基维生素D浓度将婴儿分为维生素D缺乏、不足和正常组。采用标准问卷调查形式收集父母及婴儿资料,跟踪随访婴儿至6个月龄时采用Gesell发育诊断量表进行神经发育测评,比较不同脐血维生素D水平婴儿6个月龄发育商在大运动、精细运动、社会适应、语言、个人-社交五个能区的差异。结果新生儿脐血25-羟基维生素D平均浓度为(46.7±17.3)nmol/L;6个月龄测评时,维生素D缺乏、不足和正常组3组婴儿之间的大运动、精细运动评分之间差异有统计学意义(均P <0.05);3组间两两比较,维生素D缺乏与正常组大运动、精细运动评分之间差异有统计学意义(均P <0.001 7)。脐血25-羟基维生素D水平与大运动、精细运动评分之间呈正相关(r=0.336 1、0.349 4,均P <0.001)。结论脐血25-羟基维生素D水平与婴儿6个月龄大运动、精细运动评分之间呈弱正相关,脐血25-羟基维生素D缺乏或不足对婴儿神经系统发育的潜在影响值得关注。
Objective This research aimed to investigate the effect of 25 hydroxyvitamin D in cord bloodon neurodevelopment of 6-month-old infants.MethodsA total of 142 newborns from December 2016 to December 2017 were selected prospectively. The level of 25 hydroxyvitamin D in cord blood was detected using enzyme-linkedimmunosorbent assay. According to the concentration of 25 hydroxyvitamin D in cord blood,infants were dividedinto three groups:vitamin D deficient,insufficient and normal group. Standard questionnaire was used to collectthe data of parents and infants,and Gesell developmental diagnostic scale was used for the evaluation of the neuraldevelopment of infants follow up to 6 months. The differences of 6-month-old infants with different cord bloodvitamin D levels in the five functional areas of gross motor,fine motor,adaptability,language and social skill werecompared.ResultsThe mean concentration of 25 hydroxyvitamin D in cord blood of newborn was(46.7 ± 17.3)nmol/L;at 6 months old,there was a significant difference in the scores of gross motor and fine motor among thethree groups(P < 0.05),and there was a significant difference in the scores of gross motor and fine motor betweenthe three groups(all P < 0.001 7). There was a positive correlation between the level of 25 hydroxyvitamin D in cordblood and the scores of gross motor and fine motor(R = 0.336 1,0.349 4,respectively,P < 0.001).Conclusions There was a weak positive correlation between the level of 25 hydroxyvitamin D in cord blood and 6-month-oldinfants′ gross motor and fine motor scores. The potential effect of the lack or deficiency of 25 hydroxyvitamin D incord blood on the development of nervous system in infants should be noted.
