CEA、CYFRA21-1、NSE、SCC及ProGRP联合检测对肺癌的诊断价值 被引量:8

摘要 目的研究CEA、CYFRA21-1、NSE、SCC及ProGRP联合检测对肺癌的诊断价值。方法选择肺癌患者(肺癌组)、良性肺病患者(肺良性病变组)、健康体检人员(正常对照组)各100例,应用雅培公司提供的试剂盒采用化学发光法检测CEA、CYFRA21-1、NSE、SCC及ProGRP水平。计算血清肿瘤标志物单独与联合检测肺癌的灵敏度和特异度。结果三组研究对象的CEA、CYFRA21-1、NSE、SCC及ProGRP水平比较,差异均有统计学意义(P<0.05);肺癌组CEA、CYFRA21-1、NSE、SCC及ProGRP水平均显著高于肺良性病变组和正常对照组(P<0.05);肺良性病变组CEA、CYFRA21-1、NSE、SCC及ProGRP水平均高于正常对照组(P<0.05)。腺癌患者血清CEA阳性率明显高于鳞癌和小细胞肺癌(P<0.05);鳞癌患者的血清CYFRA21-1和SCC阳性率高于腺癌和小细胞肺癌(P<0.05);小细胞肺癌患者的血清NSE和ProGRP阳性率显著高于腺癌和鳞癌(P<0.05)。五种肿瘤标记物联合检测肺癌的灵敏度明显高于各肿瘤标志物单独检测,但总体特异度相对较低。结论五种肿瘤标志物联合检测对肺癌早期明确诊断具有较高的临床应用价值,不但可提高肺癌的检出率,还有利于区分肺癌的组织分型。 Objective To study the diagnostic value of combined detection of CEA,CYFRA21-1,NSE,SCC and ProGRP in lung cancer.Methods A total of 100 lung cancer patients(lung cancer group),100 benign lung disease patients(lung benign lesion group)and 100 healthy physical examination personnel(normal control group)were selected.The CEA,CYFRA21-1,NSE,SCC and ProGRP levels were detected by chemiluminescence method with the kit provided by Abbott.The sensitivity and specificity of single and combined detection of serum tumor marker in lung cancer were calculated.Results There were significant differences in the levels of CEA,CYFRA21-1,NSE,SCC and ProGRP among the three groups(P<0.05);the CEA,CYFRA21-1,NSE,SCC and ProGRP levels in the lung cancer group were significantly higher than those in the lung benign lesion group and the normal control group(P<0.05);the CEA,CYFRA21-1,NSE,SCC and ProGRP levels in the lung benign lesion group were higher than those in the normal control group(P<0.05).The positive rate of serum CEA in patients with adenocarcinoma was significantly higher than that of squamous cell carcinoma and small cell lung cancer(P<0.05);the positive rates of serum CYFRA21-1 and SCC in patients with squamous cell carcinoma were higher than those in adenocarcinoma and small cell lung cancer(P<0.05);the positive rates of serum NSE and ProGRP in patients with small cell lung cancer were significantly higher than those in adenocarcinoma and squamous cell carcinoma(P<0.05).The sensitivity of combined detection of five tumor markers for lung cancer was significantly higher than that of single detection of tumor markers,but the overall specificity was relatively low.Conclusion Combined detection of five tumor markers has higher clinical application value for early diagnosis of lung cancer,which can not only improve the detection rate of lung cancer,but also help to distinguish the tissue type of lung cancer.
