医院-社区联动护理在独居老年高血压患者院外护理中的应用效果

Application effect of hospital-community linkage nursing in out-of-hospital nursing of elderly patients living alone with hypertension

目的探讨医院-社区联动护理在独居老年高血压患者院外护理中的应用效果。方法随机将2018年6月至2019年6月我院收治的106例独居老年高血压患者分为对照组(53例,常规院外护理)与观察组(53例,医院-社区联动护理)。比较两组的护理效果。结果干预后,两组EWB、PWB、SWB、自我概念、自护责任、自我护理技能、健康知识水平和生活质量评分均升高,且观察组高于对照组(P<0.05)。干预后,两组SBP、DBP、全血高切黏度、全血低切黏度、血浆黏度、红细胞压积均降低,且观察组低于对照组(P<0.05)。结论医院-社区联动有利于改善独居老年高血压患者的心理健康状况,提升其自护能力,促进临床相关指标改善,稳定血压,进而提高患者的生活质量。

Objective To explore the application effect of hospital-community linkage nursing in out-of-hospital nursing of elderly patients living alone with hypertension.Methods A total of 106 elderly patients living alone with hypertension admitted to our hospital from June 2018 to June 2019 were randomly divided into control group(53 cases,routine out-of-hospital nursing)and observation group(53 cases,hospital-community linkage nursing).The nursing effects of the two groups were compared.Results After intervention,the scores of EWB,PWB,SWB,self-concept,selfcare responsibility,self-care skills,health knowledge level and quality of life in the two groups improved,and those in the observation group were higher than the control group(P<0.05).After intervention,SBP,DBP,whole blood high shear viscosigy and whole blood low shear viscosity,plasma viscosity and hematocrit decreased in both groups,and those in the observation group were lower than the control group(P<0.05).Conclusion The hospital-community linkage nursing is conducive to improve the mental health state and self-care ability of elderly patients living alone with hypertension,promote the improvement of clinical related indexes,stabilize blood pressure,and improve the quality of life of patients.
