某区高危型人乳头瘤病毒感染与液基细胞学的临床分析 被引量:1

摘要 目的分析惠州地区女性高危型人乳头瘤病毒(HR-HPV)感染亚型分布特征,同时分析其与液基细胞学(TCT)的相关性。方法收集2014年1月至2018年12月在惠州市第一妇幼保健院妇科因各种原因就诊、自愿接受宫颈HR-HPV和TCT筛查,且HR-HPV筛查结果为阳性的3163例女性患者的病例资料。结果 HR-HPV亚型分布感染排名前三位分别为HPV52、HPV16和HPV58,分别占全部亚型的22.91%、15.17%和12.77%。主要为单一亚型感染,占总HR-HPV阳性病例的80.05%;二重感染和三重及以上感染分别为15.84%和4.11%。三重及以上HR-HPV感染引起的宫颈ASC-H和HSIL病变最高,分别达到10.00%和9.23%。结论惠州地区HR-HPV感染主要亚型为HPV52、HPV16和HPV58,且以单一亚型感染为主,但多重感染能使宫颈高级别病变发生率显著增加。 Objective To learn type-specific prevalence and distribution of high-risk human papillomavirus(HR-HPV) in Huizhou area, and analysis of correlation HR-HPV between thinprep cytologic test(TCT). Methods From January 2014 to December 2018, data were collected from 3,163 female patients who were admitted to the department of gynaecology of huizhou first maternal and child health hospital for various reasons and voluntarily underwent cervical HR-HPV and TCT screening, and the results of HR-HPV screening were positive. Results In terms of the distribution of HR-HPV subtypes, the top three infections were HPV52, HPV16 and HPV58, accounting for 22.91%, 15.17% and 12.77% of all subtypes, respectively. In terms of infection distribution, it was mainly a single subtype infection, accounting for 80.05% of the total HR-HPV positive cases. Double infection and triple infection were 15.84% and 4.11%, respectively. Cervical ASC-H and HSIL lesions caused by triple or more HR-HPV infection were the highest, reaching 10.00% and 9.23%, respectively. Conclusion The main subtypes of HR-HPV infection in Huizhou are HPV52, HPV16 and HPV58, and the majority are single subtype infection. However, multiple infection can significantly increase the incidence of high-grade cervical lesions..
