血涂片分析在血常规检验中的重要性

Importance of Blood Smear Analysis in Blood Routine Test

目的探究血常规检验中血涂片分析的重要作用。方法于2017年4月至2018年9月这一期间随机选取本院搜集的820例血液样本,利用血细胞分析仪对血液予以计数与分类检测,按照标准流程纸片与染色后,在显微镜下复检标本,对血细胞数量、白细胞分类以及形态进行评估,并于2 h内完成上述操作。分析血液分析检验结果,并分析正常组患者的假阴性与异常组的假阳性情况。结果 820例住院患者经血常规检验发现,正常462例,正常率57.56%;异常348例,异常率42.44%;血涂片分析检验正常血涂片分析检验发现正常463例,正常率98.09%;异常9例,假阴性率1.91%;血涂片分析检验异常血涂片分析检验发现正常163例,正常率46.84%;异常9例,假阳性率53.16%。结论血液分析仪检验结果只可作为检验的筛查与参考,血涂片检测对疾病的诊断具有指导效果;如果怀疑患者产生血液系统疾病,需及时开展血涂片检查,避免产生误诊或漏诊情况,延误患者的最佳治疗时机。

Objective To explore the important role of blood smear analysis in blood routine examination. Methods 820 blood samples collected in our hospital were randomly selected from April 2017 to September 2018. Blood was counted and classified by blood cell analyzer. Samples were rechecked under a microscope after paper sheets and staining according to the standard procedure to evaluate the number, classification and morphology of blood cells. The above operations were completed within 2 hours. Analysis of blood analysis test results, and analysis of the normal group of patients with false negative and abnormal group of false positive. Results Routine blood tests of 820 hospitalized patients showed that 462 were normal, with a normal rate of 57.56%. There were 348 abnormal cases, with an abnormal rate of 42.44%. Blood smear analysis showed that 463 cases were normal, with a normal rate of 98.09%. Abnormal 9 cases, false negative rate 1.91%;Abnormal blood smears were found in 163 cases, with a normal rate of 46.84%. Abnormal 9 cases, false positive rate was 53.16%. Conclusion Blood analyzer test results can only be used as screening and reference for testing, and blood smear test has guiding effect on disease diagnosis. If the patient is suspected to have blood system diseases, blood smear examination should be carried out in time to avoid misdiagnosis or missed diagnosis and delay the best treatment time for the patient.
