康柏西普联合532nm激光光凝治疗缺血型视网膜分支静脉阻塞继发黄斑水肿的疗效观察 被引量:8

Curative effects of conbercept combined with 532 nm laser photocoagulation on macular edema secondary to ischemic branch retinal vein occlusion

摘要 目的探讨康柏西普联合532 nm激光光凝对缺血型视网膜分支静脉阻塞(BROV)继发黄斑水肿患者的治疗效果。方法选取2017年1月至2019年1月在新乡医学院第三附属医院就诊的缺血型BROV继发黄斑水肿患者88例(88只眼)纳入研究,采用随机数字表法分为两组,每组44例(44只眼)。研究组给予康柏西普联合532 nm激光光凝治疗,对照组给予532 nm激光光凝治疗。比较两组治疗前后最佳矫正视力(BCVA)、黄斑中心视网膜厚度(CMT)变化、视力变化及并发症发生情况。结果两组治疗后BCVA均高于治疗前,其中治疗后1周、1个月,研究组BCVA值高于对照组,差异有统计学意义(P<0.05)。两组治疗后CMT值均低于治疗前,其中治疗后1周、1个月,研究组CMT低于对照组,差异有统计学意义(P<0.05)。研究组激光光凝术中房水中血管内皮生长因子(VEGF)浓度低于对照组(P<0.05)。研究组并发症发生率[4.55%(2/44)]与对照组[11.36%(5/44)]比较,差异未见统计学意义(P>0.05)。结论康柏西普联合532 nm激光光凝用于治疗缺血型BROV继发黄斑水肿,可提高患者矫正视力,降低CMT和房水中VEGF浓度。 Objective To investigate curative effects of conbercept combined with 532 nm laser photocoagulation on patients with macular edema secondary to ischemic branch retinal vein occlusion(BROV).Methods Eighty-eight patients(88 eyes)with macular edema due to ischemic BROV treated in the Third Affiliated Hospital of Xinxiang Medical University from January 2017 to January 2019 were selected and divided into two groups by random number table method,with 44 patients(44 eyes)in each group.Patients in the research group were treated with conbercept combined with 532 nm laser photocoagulation,while patients in the control group were treated with 532 nm laser photocoagulation.The changes in best corrected visual acuity(BCVA),central macular thickness(CMT)and visual acuity before and after treatment,and incidences of complications were compared between the two groups.Results After treatment,BCVA values of the two groups were increased,and BCVA value of research group was higher than that of control group 1 week and 1 month after treatment(P<0.05).After treatment,CMT values of the two groups were decreased,and the index of research group was significantly lower than that of control group 1 week and 1 month after treatment(P<0.05).During performance of laser photocoagulation,concentration of vascular endothelial growth factor(VEGF)in aqueous humor of the research group was lower than that of the control group(P<0.05).There was no difference in complication rate between research group(4.55%,2/44)and control group(11.36%,5/44),P>0.05.Conclusions Conbercept combined with 532 nm laser photocoagulation in treatment of macular edema secondary to ischemic BROV can improve corrected visual acuity,reduce CMT and concentration of VEGF in aqueous humor.
