红霉素与阿奇霉素序贯疗法治疗小儿支原体肺炎的疗效及不良反应 被引量:5

Efficacy and adverse reactions of erythromycin and sequential therapy azithromycin in the treatment of mycoplasma pneumonia in children

摘要 目的探讨红霉素与阿奇霉素序贯结合对小儿支原体肺炎的治疗价值及不良反应。方法选取聊城市人民医院2018年2月至2019年2月收治的86例支原体肺炎患儿为实验对象,依据不同的治疗方法将入选患儿分为观察组与对照组,每组43例。对照组行红霉素治疗,观察组在红霉素的基础上加用阿奇霉素序贯疗法治疗。观察两组患儿的疗效、症状改善情况、不良反应及其治疗前后的患儿潮气量(V-T)、第一秒用力呼气容积(FEV1)、最大呼气流量(PEF)等肺功能指标变化情况。结果治疗后,对照组总有效率(72.09%)与观察组(97.67%)比较,差异有统计学意义(χ^2=10.965,P<0.05)。观察组患者的退热时间为(3.60±0.65)d,低于对照组[(5.97±1.10)d],P<0.05;观察组咳嗽消失时间为(3.31±1.09)d,低于对照组[(7.06±1.42)d],P<0.05;观察组啰音消失时间为(5.16±1.32)d,低于对照组[(9.02±2.04)d],P<0.05;观察组住院时间为(10.90±2.66)d,低于对照组[(12.42±3.86)d],P<0.05。观察组的V-T、FEV1、PEF显著低于对照组,差异有统计学意义(P<0.05)。观察组总不良反应率(13.95%)与对照组(32.56%)比较,差异有统计学意义(χ^2=4.170,P<0.05)。结论红霉素与阿奇霉素序贯疗法治疗小儿支原体肺炎中具有较高临床价值,可缩短症状改善时间,改善患儿肺功能,且不良反应少。 Objective To investigate the effect and adverse reactions of erythromycin and sequential azithromycin on mycoplasma pneumonia in children.Methods A total of 86 children with mycoplasma pneumonia admitted to Liaocheng People’s Hospital from February 2018 to February 2019 were selected as the experimental subjects,and the selected children were divided into observation group and control group according to different treatment methods,with 43 cases in each group.The control group was treated with erythromycin,and the observation group was treated with azithromycin sequential therapy on the basis of erythromycin.The effect,improvement of symptoms,adverse reactions and the changes of plumonary function index such as tidal volume(V-T),the first second forced exhalation volume(FEV1),and peak expiratory flow(PEF)before and after treatment of the two groups were compared.Results After treatment,the total effective rate of the control group(72.09%)was compared with 97.67%of the observation group,there was significant difference between the two groups(χ^2=10.965,P<0.05).The time of fever reduction[(3.60±0.65)d]in the observation group was significantly lower than[(5.97±1.10)d]in the control group,P<0.05;the time of cough disappearance[(3.31±1.09)d]in the observation group was significantly lower than[(7.06±1.42)d]in the control Group,P<0.05;the time of rale disappearance[(5.16±1.32)d]in the observation group was significantly lower than that in the control group[(9.02±2.04)d],P<0.05;the time of hospitalization[(10.90±2.66)d]in the observation group was significantly lower than[(12.42±3.86)d]in the control group,P<0.05.The V-T,FEV1 and PEF in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).The total adverse reaction rate of the observation group(13.95%)was compared with that of the control group(32.56%),the difference was statistically significant(χ^2=4.170,P<0.05).Conclusions Erythromycin and azithromycin sequential therapy have high clinical value in mycoplasma pneumonia,which can significantly shorten the time of symptom improvement,improve the lung function of children,and have less adverse reactions.
