革兰氏阴性菌感染患者外周血中性粒细胞CD64指数变化及意义

Changes of CD64 index of neutrophils in patients with Gram-negative bacteria infection

目的探讨革兰氏阴性菌感染患者外周血中性粒细胞CD64指数变化。方法收集蚌埠市第一人民医院2018年8月至2019年1月革兰氏阴性菌感染患者35例,健康对照组35例,采用流式细胞术分别检测35例经血培养确定为阳性的革兰氏阴性菌感染患者及健康对照组人群外周血中性粒细胞CD64表达,血细胞分析仪检测外周血白细胞总数和中性粒细胞百分比,用特定蛋白分析仪检测外周血C反应蛋白,化学发光法检测血清降钙素原,并对两组数据进行比较和分析。结果与对照组相比,35例革兰氏阴性菌感染患者外周血中性粒细胞CD64指数[(52.25±21.63)比(1.23±0.68)]、白细胞总数[(12.41±7.21)×10^9/L比(7.85±1.14)×10^9/L]、中性粒细胞百分比[(76.35±21.14)%比(67.66±6.08)%]、C反应蛋白[(59.91±17.04)mg/L比(3.34±1.48)mg/L]和降钙素原[(27.03±12.65)ng/L比(0.15±0.02)ng/L]都明显高于健康对照组,差异具有统计学意义(P值均<0.05)。结论外周血中性粒细胞CD64指数可作为临床鉴别细菌感染提供参考依据。

Objective To investigate the change of CD64 index of peripheral blood neutrophils in patients with Gram-negative bacterial infection.Methods All of the 35 cases of Gram-negative bacteria infection from August 2018 to January 2019 in the first people's Hospital of Bengbu were collected as experimental group, and 35 cases of healthy were control group.Flow cytometry was used to detect CD64 in peripheral blood neutrophils of 35 patients with Gram-negative bacterial infection identified as positive by blood culture.The total number of white blood cells and percentage of neutrophils in peripheral blood were detected by Hematology analyzer.C-reactive protein in peripheral blood was detected by specific protein analyzer.Serum procalcitonin was detected by chemiluminescence method.At the same time, 35 healthy controls were detected.Peripheral blood neutrophils CD64, total white blood cells, percentage of neutrophils, C-reactive protein and procalcitonin, and CD64 infection index was calculated by flow analysis software.Results Compared to the control group, the indicators[CD64 index(52.25±21.63)vs(1.23±0.68), leukocyte count(12.41±7.21)×10^9/L vs (7.85±1.14)×10^9/L, neutrophil percentage(76.35±21.14)% vs (67.66±6.08)%, C-reactive protein(59.91±17.04)mg/L vs(3.34±1.48)mg/L and calcitonin(27.03±12.65)ng/L vs(0.15±0.02)ng/L]in peripheral blood of 35 patients with Gram-negative bacterial infection were significantly higher (all P<0.05).ConclusionCD64 index of peripheral blood neutrophils may be used as a reference for clinical identification of bacterial infections.
