盐酸纳美芬治疗急性酒精中毒的临床效果及对神经递质水平的影响 被引量:23

Clinical effects of nalmefene hydrochloride and its effects on neurotransmitters levels in patients with acute alcoholism

摘要 目的探讨盐酸纳美芬对急性酒精中毒的临床效果及对神经递质水平的影响。方法选择2018年1月至2019年6月于黄石市中心医院普爱院区急诊科收治的急性酒精中毒患者134例为研究对象,采用随机数字表法将患者分为观察组和对照组,各67例。对照组采用盐酸纳洛酮治疗,观察组采用盐酸纳美芬治疗。比较2组患者治疗总有效率、症状消失时间、苏醒时间、住院时间、神经递质和药物不良反应的差异。结果观察组患者的治疗总有效率为94. 03%,明显高于对照组(77. 61%);且观察组的症状消失时间、苏醒时间和住院时间均明显少于对照组(P<0. 05)。2组患者接受治疗后,观察组患者血清5-羟色胺、去甲肾上腺素、多巴胺、一氧化氮、血管加压素、β-内啡肽水平较对照组降低幅度更明显(P<0. 05)。同时,观察组患者药物不良反应的发生率为5. 97%,明显低于对照组(22. 39%),差异具有统计学意义(P<0. 05)。结论盐酸纳美芬治疗急性酒精中毒的临床疗效更满意,可有效减轻对神经递质水平的影响,且药物安全性较高,值得临床推广应用。 Objective This study was designed to investigate the clinical effect of nalmephen hydrochloride on acute alcoholism and its effect on neurotransmitter levels. Methods 134 patients with acute alcoholism admitted to emergency department of Huangshi Central Hospital Puai Branch Hospital from January 2018 to June 2019 were selected as the study subjects.Patients were randomly divided into the research group and the control group were 67 cases in each. The patients in the control group were treated with naloxone hydrochloride. The patients in the study group were treated with nalmefene hydrochloride.Total effective rate,symptom disappearance time,recovery time,hospital stay length,neurotransmitters and adverse drug reactions were compared between the two groups. Results The clinical efficacy of the study group was significantly better than that of the control group,and the total effective rate(94. 03%)was significantly higher than that of the control group(77. 61%).After treatment of the two groups,serum 5-hydroxytryptamine,norepinephrine,dopamine,nitric oxide,vasopressin and β-endorphin levels in the research groups were significantly better than the control group(P<0. 05). The adverse reactions rate in the study group(5. 97%)was significantly lower than that control group(22. 39%,P<0. 05). Conclusion The clinical efficacy of nalmefene hydrochloride in patients with acute alcoholism is satisfying,it can effectively reduce neurotransmitter levels,and has good safety,which is worthy of clinical application.
