潮气呼吸肺功能在反复喘息婴幼儿疾病管理中的应用研究 被引量:2

摘要 目的:探讨潮气呼吸肺功能在反复喘息婴幼儿疾病控制及管理中的应用价值。方法:将2017年7月-2018年7月在广东省珠海市妇幼保健院院儿科门诊因反复喘息就诊的婴幼儿60例根据哮喘预测指数(API)分为API阳性组(n=30)和API阴性组(n=30),比较其临床表现、首诊肺功能的差异,并根据不同组别定期测量肺功能,比较治疗后的变化。结果:API阳性组患儿的个人过敏性疾病及过敏性家族史的阳性率明显高于API阴性组,但两组均高于健康人群(P均<0.05)。API阳性组的首诊肺功能达峰时间比(TPTEF/TE)及达峰容积比(VPEF/VE)明显差于API阴性组(P<0.05)。治疗1月、3月后肺功能指标均有改善,其中TPTEF/TE及VPEF/VE有统计学差异(P<0.05),治疗3月及12月相比肺功能有改善,但无统计学差异。结论:潮气呼吸肺功能联合哮喘预测指数在反复喘息婴幼儿的诊治和疾病管理中有较大应用价值。 Objective:To explore the application of tidal lung function to disease control and management for infants with recurrent wheezing. Methods:60 cases of asthmatic patients were divided into API positive group(n=30)and API negative group(n=30)in terms of asthma predictive index(API)from Jun 2017 to Jun 2018.The Clinical manifestations and pulmonary function were compared at the first diagnosis. Pulmonary function was measured regularly respectively and the changes after treatment were compared between the two groups. Results:The positive rate of individual allergic diseases and family allergic history in the API positive group were significantly higher than those in the API negative group(P<0.05).The peak time ratio(TPTEF/TE) and peak volume ratio(VPEF/VE) in the API positive group were worse than those in the API negative group,but both were lower than those in the healthy control group(all P<0.05). Follow-up of the two groups showed that lung function indexes improved after 1 and 3 months of treatment,among which TPTEF/TE and VPEF/VE showed statistically differences(P<0.05),and the lung function improved after 3 and 12 months after treatment,but there was no statistically significant difference. Conclusion:The predictive index of moist breath and lung function combined with asthma is of great value in the diagnosis,treatment and disease management for infants with recurrent wheezing.
