微创拔牙技术在下颌埋伏阻生智齿拔除过程中的应用研究 被引量:44

Application of minimally invasive tooth extraction technology in extraction of mandibular unerupted-impacted wisdom teeth.

摘要 目的:探讨微创拔牙技术在下颌埋伏阻生智齿拔除过程中的应用价值。方法:选取2018年3月~2019年1月在本院进行下颌埋伏阻生智齿拔除术的患者80例,根据手术方法不同分为研究组和对照组,每组各40例。对照组采用传统拔牙技术,研究组采用微创拔牙技术。比较两组患者骨愈合时间、拔牙时间、术后疼痛程度、并发症发生率。同时比较两组患者手术前、术后龈沟液中炎症介质、氧化应激介质、疼痛介质含量。结果:与对照组比较,研究组骨愈合时间短、拔牙时间短、疼痛程度较轻、并发症发生率低(P<0.05)。研究组治疗后白介素-1β(interleukin-1β,IL-1β)、白介素-6(interleukin-6,IL-6)、丙二醛(malondialdehyde,MDA)、P物质(substance P,SP)、5-羟色胺(5-hydroxytryptamine,5-HT)、降钙素基因相关肽(calcitonin gene-related peptide,CGRP)水平均明显低于对照组治疗后;超氧化物歧化酶(superoxide dismutase,SOD)、谷胱甘肽过氧化酶(glutathione peroxidase,GP-x)水平均明显高于对照组治疗后,差异均有统计学意义(P<0.05)。结论:微创拔牙技术在下颌埋伏阻生智齿拔除过程中,能够减轻局部炎症反应和应激反应,降低疼痛程度。 Objective:To explore the application of minimally invasive tooth extraction technology in extraction of mandibular unerupted-impacted wisdom teeth.Methods:Totally 80 patients who underwent extraction of impacted mandibular wisdom teeth in our hospital from March 2018 to January 2019.The patients were divided into study group and control group according to different surgical methods,with 40 patients in each group.The control group was treated with traditional tooth extraction technology,while the study group with minimally invasive tooth extraction technology.Bone healing time,tooth extraction time,postoperative pain and complication rate were compared.The content of inflammatory mediators,oxidative stress mediators and pain mediators in gingival crevicular fluid before and after surgery were compared between the two groups.Results:Compared with the control group,the bone healing time of the study group was short,tooth extraction time was short,the degree of pain was mild,and the incidence of complications was low(P<0.05).The levels of interleukin-1β(IL-1),interleukin-6(IL-6),malondialdehyde(MDA),substance P(SP),5-hydroxytryptamine(5-HT)and calcitonin gene-related peptide(CGRP)in the study group were significantly lower than those in the control group after treatment,and the levels of superoxide dismutase(SOD)and glutathione peroxidase(GP-x)were significantly higher than those in the control group after treatment,with statistically significant differences(P<0.05).Conclusion:Minimally invasive tooth extraction technique can reduce local inflammatory response and stress response and inhibit pain during extraction of impacted mandibular wisdom teeth.
