和胃止痛颗粒联合耳穴压豆治疗180例Hp感染慢性萎缩性胃炎的疗效观察

Clinical observation on Hewei Zhitong granule combined with auricular acupoint pressing bean in treating Hp positive chronic atrophic gastritis

[目的]观察和胃止痛颗粒联合耳穴压豆治疗Hp感染慢性萎缩性胃炎的临床疗效。[方法]收集就诊的180例Hp感染的慢性萎缩性胃炎患者,分为治疗组和对照组,每组90例,疗程均为4周。治疗组采用标准铋四联+和胃止痛颗粒口服,配合耳穴压豆治疗;对照组单纯给予杀菌治疗,4周后观察临床疗效。[结果]治疗组Hp根除率(93.33%)高于对照组(71.11%),差异有统计学意义(P<0.01);治疗组总有效率为95.56%,明显高于对照组的83.33%(P<0.01)。治疗后2组患者中医症状积分均较治疗前下降,治疗组症状积分低于对照组(P<0.01);2组患者胃蛋白酶原(PG)Ⅰ、PGⅠ/PGⅡ、血清胃泌素(G-17)水平均较治疗前升高,PGⅡ水平较治疗前下降(P<0.01),且治疗组PGⅠ、PGⅠ/PGⅡ、G-17水平均高于对照组(P<0.01);胃镜下胃黏膜的形态改善,在黏膜色泽、黏膜皱襞、黏膜颗粒状改变等方面,观察组优于对照组。[结论]和胃止痛颗粒联合耳穴压豆治疗Hp感染的慢性萎缩性胃炎有较好疗效,可以明显提高血清中PGⅠ、PGⅠ/PGⅡ、G-17的水平,进而改善胃黏膜功能,提高Hp根除率。

[Objective]To observe the clinical effect of traditional Chinese medicine Hewei Zhitong Keli combined with auricular points on chronic atrophic gastritis of Helicobacter pylori(Hp)infection.[Methods]One hundred and eighty patients with chronic atrophic gastritis infected by Hp were recruited in our hospital.These patients were randomly divided into a treatment group and a control group,with 90 patients in each group.The course of treatment was 4 weeks.The treatment group was treated with standard bismuth quadruple combined with Hewei Zhitong Keli orally,combined with auricular acupoint pressing beans.The control group was given sterilization treatment only,and the clinical effect was observed after 4 weeks.[Results]The clearance rate of Hp in the observation group(93.33%)was higher than that in the control group(71.11%),and the difference was statistically significant(P<0.01);the total effective rate in the observation group was 95.56%,which was significantly higher than the control group’s 77.78%.(P<0.01).After treatment,the scores of medical symptoms in both groups were lower than before treatment.The level of symptom score in the observation group was lower than that in the control group(P<0.01);the levels of PGⅠ,PGⅠ/PGⅡ,and G-17 in the two groups were higher than before treatment,and the PGⅡlevel was lower than before treatment(P<0.01).The levels of PGI,PGⅠ/PGⅡ,and G-17 in the observation group were higher than those in the control group(P<0.01);the eradication rate of Hp in the observation group was higher than that in the control group(P<0.01).Electronic fiber gastroscope showed an improvement in the morphology of gastric mucosa.The treatment group was superior to the control group in terms of mucosal color,mucosal folds,and mucosal granular changes.[Conclusion]Hewei Zhitong Keli combined with auricular acupoint pressing beans has a good curative effect on Hp-positive chronic atrophic gastritis,which can obviously improve the level of PGⅠ,PGⅠ/PGⅡand G-17 in the serum,and then ameliorate the function of gastric mucosa so that to increase the eradication rate of Hp.
