E钙黏着素与波形蛋白在结直肠癌中的表达情况及对患者预后的影响

Expression of E-cadherin and vimentin in colorectal cancer and its effect on prognosis

目的探讨E钙黏着素与波形蛋白在结直肠癌中的表达情况及对患者预后的影响。方法回顾性分析171例结直肠癌患者的临床资料,采用免疫组化方法检测癌变组织E钙黏着素及波形蛋白的表达,随访所有患者预后,分析E钙黏着素及波形蛋白的表达与患者临床病理特征之间的关系及两者不同表达量与患者预后的关系。结果171例患者中E钙黏着素阳性表达137例,阳性表达率80.12%,波形蛋白表达阳性者21例,阳性表达率为12.28%。E钙黏着素表达阳性患者(CEA)水平显著升高、肿瘤分化程度低及淋巴结转移与阴性表达患者比较,差异有统计学意义(P<0.05)。波形蛋白表达阳性患者年龄、CEA水平及肿瘤直径显著大于阴性表达患者,差异有统计学意义(P<0.05)。E钙黏着素表达阳性患者及波形蛋白表达阳性患者累积生存率均显著低于阴性表达患者,差异有统计学意义(P<0.05)。结论E钙黏着素及波形蛋白在结直肠癌的发生发展中发挥了重要作用,检测其表达情况可作为判断结直肠癌是否转移及预后的重要指标。

Objective To discuss the expression of E-cadherin and vimentin in colorectal cancer and its effect on prognosis.Methods The clinical data of 171 patients with colorectal cancer were retrospectively analyzed.Immunohistochemical methods were used to detect the expression of e-cadherin and vimentin in cancerous tissues.The prognosis of all patients was followed up.The correlation between the expression of e-cadherin and vimentin and the clinicopathological features of patients and the correlation between the different expression levels of the two indicators and the prognosis of patients were analyzed.Results Among 171 patients,137 cases showed positive expressions of E-cadherin,with a positive expression rate of 80.12%,and 21 cases showed positive expression of vimentin expression,with a positive expression rate of 12.28%.The carcinoembryonic antigen(CEA)level of patients with positive expression of E-cadherin was higher than that of patients with negative expression of E-cadherin,tumor differentiation was lower than that of patients with negative expression of E-cadherin,and lymph node metastasis was lower than that of patients with negative expression of E-cadherin,and the difference was statistically significant(P<0.05).The age,CEA level and tumor diameter of that of patients with positive expression of vimentin were significantly greater that those of patients with negative expression of vimentin,and the difference was statistically significant(P<0.05).The cumulative survival rate of patients with positive expression of E-cadherin and vimentin was significantly lower than that of patients with negative expression of E-cadherin and vimentin,and the difference was statistically significant(P<0.05).Conclusion E-cadherin and vimentin play an important role in the occurrence and development of colorectal cancer,and the detection of their expression can be used as an important indicator to judge whether colorectal cancer has metastasis and prognosis.
