家庭镜像疗法联合康复训练对偏瘫型脑性瘫痪儿童上肢运动功能的疗效研究

Effect of family mirror therapy combined with rehabilitation training on upper limb motor function in children with hemiplegic cerebral palsy

目的探讨家庭镜像疗法联合康复训练对偏瘫型脑性瘫痪儿童上肢运动功能的疗效。方法选取2017年6月至2017年12月海南省人民医院收治的66例偏瘫型脑性瘫痪患儿为研究对象,将其随机分为观察组和对照组,每组各33例。对照组患儿给予常规康复训练,观察组患儿在此基础上联合应用家庭镜像疗法,两组患儿均干预8周。比较两组患儿干预前后的前臂旋后角度、握力、精细运动能力测试量表(fine motor function measure,FMFM)、改良Ashworth量表(modified Ashworth scale,MAS)评分变化。结果干预后,两组患儿前臂旋后角度、握力均显著大于干预前(均P<0.05),FMFM评分均显著高于干预前(均P<0.05),MAS评分均显著低于干预前(均P<0.05)。观察组患儿干预后前臂旋后角度、握力均显著大于对照组(均P<0.05),FMFM评分显著高于对照组(P<0.05),MAS评分显著低于对照组(P<0.05)。结论家庭镜像疗法能显著提高偏瘫型脑性瘫痪患儿的上肢肌力与活动范围,降低肌张力,改善精细运动能力,应用效果令人满意。

Objective To investigate the effect of family mirror therapy combined with rehabilitation training on upper limb motor function in children with hemiplegic cerebral palsy.Method 66 children with hemiplegic cerebral palsy admitted to Hainan General Hospital from June 2017 to December 2017 were randomly divided into observation group and control group,33 cases in each group.The children in control group were given routine rehabilitation training,while children in observation group were combined with family mirror therapy on this basis.Both groups were treated for 8 weeks.The changes of forearm supination angle,grip strength,fine motor function measure(FMFM)and modified Ashworth scale(MAS)scores before and after intervention were compared between the two groups.Result After intervention,the forearm supination angle and grip strength of the two groups were significantly greater than before intervention(all P<0.05),FMFM scores were significantly higher than that before intervention(all P<0.05),and MAS scores were significantly lower than that before intervention(all P<0.05).After intervention,the forearm supination angle and grip strength in observation group were significantly greater than those in control group(all P<0.05),FMFM score was significantly higher than that in control group(P<0.05),and MAS score was significantly lower than that in control group(P<0.05).Conclusion Family mirror therapy could significantly increase the upper limb muscle strength and range of motion of children with hemiplegic cerebral palsy,reduce the muscle tension and improve the ability of fine movement,whose application effect is satisfactory.
