激励式心理疏导对原发性肝癌经皮穿刺射频消融术后患者心理状态、睡眠质量的影响

Effect of incentive psychological counseling on postoperative psychological state and sleep quality in patients with primary hepatic carcinoma receiving percutaneous radiofrequency ablation

目的探讨激励式心理疏导对原发性肝癌经皮射频消融术后患者心理状态和睡眠质量的改善效果。方法采用随机数字表法将110例行经皮穿刺射频消融术的原发性肝癌患者分为研究组和对照组,每组55例。术后,研究组采用常规护理+激励式心理疏导,对照组采用常规护理干预。比较两组患者干预前后的焦虑自评量表(SAS)、抑郁自评量表(SDS)评分和匹兹堡睡眠质量指数(PSQI)。结果干预后,两组患者的SAS、SDS评分均低于本组干预前,差异均有统计学意义(P<0.05);干预后,研究组患者的SAS、SDS评分均低于对照组患者,差异均有统计学意义(P<0.05)。干预后,两组患者的睡眠质量、入睡时间、睡眠效率、睡眠时间、睡眠障碍、日间睡眠障碍、催眠药物使用情况维度的评分及PSQI评分均低于本组干预前,差异均有统计学意义(P<0.05);干预后,研究组患者入睡时间、睡眠效率、睡眠时间、睡眠障碍维度的评分及PSQI总分均低于对照组,差异均有统计学意义(P<0.05)。结论激励式心理疏导对原发性肝癌经皮穿刺射频消融术后患者的心理状态、睡眠质量具有改善作用。

Objective To explore the effect of incentive psychological counseling on the improvement of postoperative psychological state and sleep quality in patients with primary hepatic carcinoma receiving percutaneous radiofrequency ablation.Method A total of 110 patients with primary hepatic carcinoma who underwent percutaneous radiofrequency ablation were selected and divided into the study group and control group according to random number table.Each group contained 55 subjects.The study group was given routine care combined incentive psychological counseling while the control group was given routine care only.Self-rating anxiety scale(SAS) score,self-rating depression scale(SDS)score,Pittsburgh sleep quality index(PSQI) before and after intervention were compared between the two groups.Result The SAS scores and SDS scores in both groups after the intervention were lower than those before the intervention,and the differences were statistically significant(P<0.05).After the intervention,the SAS scores and SDS scores of the study group were lower than of the control group,and the differences were statistically significant(P<0.05).After the intervention,the scores of sleep quality,time to fall asleep,sleep efficiency,sleep time,sleep disorder,daytime sleep disorder,hypnotic drug use and the PSQI scores of the two groups were lower than those of the groups before the intervention,and the differences were statistically significant(P<0.05).After the intervention,the scores of time to fall asleep,sleep efficiency,sleep time,sleep disorder,as well as the total scores of PSQI were lower than that in control group and the differences were statistically significant(P<0.05).Conclusion Incentive psychological counseling could significantly improve the postoperative psychological state and sleep quality of primary hepatic carcinoma patients undergoing percutaneous radiofrequency ablation.
