摘要
目的测评天津市某养老院老年人人体成分数据与认知功能评分,分析人体成分特点与老年人认知功能评分之间的相关性,为预防和控制认知障碍提供参考。方法(1)采用整群随机抽样法,对象为2014年至2017年在天津市某养老机构的229名老年人,采集人体成分数据,通过场调查问卷、简易精神状态量表和日常生活能力量表等评价量表的方式,全面评价老人的营养和认知状况。(2)采用1:3配对的巢式病例-对照研究设计。随机抽取年龄、学历背景,MMSE、BMI相同的认知功能正常的老人进入对照组,病例组根据MMSE评分进行病例组筛选。结果229例老年人,其中病例组13人(女性9人,占比69.2%),年龄为81±10.76岁;对照组156人(女性97人,占比62.2%),年龄为77.91±7.45岁。斯皮尔曼相关系数显示体脂肪率、体脂肪量、非脂肪量、水分含量与认知水平均无相关性。结论结论巢式病例-对照研究尚不能得出老年人的体脂肪量与MMSE有相关性,需扩大样本量进行验证。
Objective To examine the correlations between cognitive function and body composition in the elderly population in a nursing home.Methods(1)The data on cognitive function and body composition of 229 residents living in a nursing home in Tianjin between 2014 and 2017 was collected using the random cluster sampling method.The cognitive function of these subjects was assessed using questionnaires,the Minimum Mental State Examination(MMSE),and Activities of Daily Living(ADL)Scale.Bioelectrical impedance analysis(BIA)was used to estimate their body composition.Anthropometric measurements and laboratory data were collected in clinical settings.(2)A nested case-control study(1:3)was conducted.Each participant in the control group was matched to the others in age,education levels,MMSE scores and BMI.Results A total of 229 elderly patients were recuited,including 13 subjects(9 females,69.2%)in the case group with an age of 81±10.76 years and 156 subjects(97 females,62.2%)in the control group with an age of 77.91±7.45 years.Spearman's rho showed no correlations between BFP,FM,FFM and TBW.Conclusion Nested case-control study shows no significant correlation between body composition and MMSE in the elderly.Further study is needed in an expanded population.
