子宫内膜息肉患者生殖道解脲支原体、人型支原体感染情况分析及其诊断价值

Analysis of the infection situation of ureaplasma urealyticum and mycoplasma hominis in the genital tract of patients with endometrial polyps and their diagnostic value

目的探讨子宫内膜息肉患者生殖道解脲支原体、人型支原体感染情况,及两者在EP中的诊断价值。方法选取2019年1月~2020年1月我院收治的EP患者100例为观察组,根据宫腔镜结合经阴道彩色多普勒检查和病理检查结果,将观察组分为良性亚组(n=89)和恶性亚组(n=11)。同期就诊未见EP的体检者100例为对照组。取两组宫颈上皮细胞,进行实时定量荧光PCR,分别检测并分析两组UU、MH感染情况。受试者工作特征曲线(ROC)分析UU、MH在EP中的诊断价值。结果观察组UU、MH感染检出率及UU+MH检出率均高于对照组(P<0.05)。观察组UU、MH感染水平均显著高于对照组(P<0.05)。观察组中恶性亚组的UU、MH感染水平均高于良性亚组(P<0.05)。ROC曲线结果显示,UU、MH诊断EP的曲线下面积(AUC)分别为0.674(95%CI:0.588~0.955)、0.658(95%CI:0.606~0.960),两者联合诊断EP的AUC为0.725(95%CI:0.694~1.000)。结论EP患者生殖道存在UU、MH感染,UU、MH对EP的单独诊断价值较低,两者联合诊断价值更高。

Objective To investigate the infection situation of ureaplasma urealyticum(UU)and mycoplasma hominis(MH)in the genital tract of patients with endometrial polyps(EP)and their diagnostic value in EP.Methods 100 EP patients admitted to our hospital from January 2019 to January 2020 were selected as the observation group.According to the results of hysteroscopy combined with transvaginal color Doppler examination and pathological examination,the observation group was divided into the benign subgroup(n=89)and the malignant subgroup(n=11).In the same period,100 patients without EP were taken as the control group.Two groups of cervical epithelial cells were taken for real-time quantitative PCR,in which UU and MH infections were detected and analyzed respectively.The diagnostic values of UU and MH in EP were analyzed by receiver operating characteristic(ROC)curve.Results The detection rates of UU and MH infection and UU combined with MH infection in the observation group were higher than those in the control group(P<0.05).The infection levels of UU and MH in the observation group were significantly higher than those in the control group(P<0.05).In the observation group,UU and MH infection levels in the malignant subgroup were higher than those in the benign subgroup(P<0.05).ROC curve results showed that AUC of UU and MH diagnosis EP were 0.674(95%CI 0.588-0.955)and 0.658(95%CI 0.606-0.960),respectively,and AUC of combined diagnosis EP was 0.725(95%CI 0.694-1.000).Conclusion UU and MH infections exist in the genital tract of EP patients.UU and MH have lower diagnostic value for EP alone and higher diagnostic value in combination.
