下腔静脉滤器置入+置管溶栓+足背静脉溶栓疗法治疗急性下肢深静脉血栓的效果分析

Analysis of the effect of implantation of inferior vena cava filter+cat heter thrombolysis+dorsal foot venous thrombolysis in the treatment of acute lower extremity deep ven ous thrombosis

目的:探讨用下腔静脉滤器置入+置管溶栓+足背静脉溶栓疗法治疗急性下肢深静脉血栓的临床效果。方法:选择东莞东华医院2017年1月至2019年7月收治的急性下肢深静脉血栓患者60例作为研究对象。采用随机数表法将其分为比较组和分析组(30例/组)。为分析组患者采用下腔静脉滤器置入+置管溶栓+足背静脉溶栓疗法进行治疗,为比较组患者采用下腔静脉滤器置入+足背静脉溶栓疗法进行治疗。然后比较两组患者治疗的总有效率、治疗前后其双侧大腿的周径差和双侧小腿的周径差。结果:分析组患者治疗的总有效率(96.67%)高于比较组患者治疗的总有效率(76.67%),P<0.05。治疗后,分析组患者双侧大腿的周径差〔(1.2±0.9)cm〕、双侧小腿的周径差〔(0.8±0.2)cm〕均小于比较组患者双侧大腿的周径差〔(3.0±0.5)cm〕、双侧小腿的周径差〔(1.9±0.4)cm〕,P<0.05。结论:对急性下肢深静脉血栓患者采用下腔静脉滤器置入+置管溶栓+足背静脉溶栓疗法进行治疗的效果较好,可显著减小其双侧大腿、小腿的周径差。

Objective:To investigate the clinical effect of implantation of inferior vena cava filter+catheter thrombolysis+dorsal foot venous thrombolysis in the treatment of acute lower extremity deep venous thrombosis(DVT).Methods:60 patients with acute deep venous thrombosis of lower extremities admitted to Dongguan Donghua Hospital from January 2017 to July 2019 were selected as study subjects.They were divided into comparison group and analysis group(30 cases/group)by random number table method.Patients in the analysis group were treated with inferior vena cava filter implantation+catheter thrombolysis+dorsal foot vein thrombolysis therapy,while patients in the comparison group were treated with inferior vena cava filter im plantation+dorsal foot vein thrombolysis therapy.Then the total effective rate of the two groups of patients before and after treatment,the circumference difference of bilateral thighs and the circumference difference of bilateral calves were compared.Results:The total effective rate(96.67%)in the analysis group was higher than that in the comparison group(76.67%),P<0.05.After treatment,the difference between the two legs((1.2±0.9)cm)and the difference between the two legs((0.8±0.2)cm)in the analysis group was smaller than that between the two legs((3.0±0.5)cm)and the difference between the two legs(1.9±0.4)cm in the comparison group(P<0.05).Conclusion:Implantation of inferior vena cava filter+catheter thrombolytic therapy+dorsal foot venous thrombolytic therapy in patients with acute lower extremity deep vein thrombosis has a good therapeutic effect,and can significantly reduce the circumference difference of bilateral thighs and calves.
