丁卡因气道表面麻醉在气管插管全麻手术中的应用

摘要 目的:观察用丁卡因对接受全麻手术的患者进行气道表面麻醉在减轻其心血管应激反应中的效果。方法:选择2019年3月至2019年8月期间在中山大学孙逸仙纪念医院南院择期进行全麻手术的50例患者为研究对象。将其随机分为对照组和观察组。在对两组患者进行麻醉诱导后,对对照组患者进行常规的气管插管,对观察组患者先用丁卡因进行气道表面麻醉,再进行气管插管。手术结束后,对比两组患者在进入手术室后不同时间的心率(HR)和平均动脉压(MAP)。结果:在进入手术室安静平卧5 min后(T1)、进行气管插管前(T2),两组患者的HR、MAP相比,P>0.05。在进行气管插管后1 min内(T3)、拔管后1 min内(T4),观察组患者的HR、MAP均低于对照组患者,P<0.05。在T3,对照组患者的HR、MAP均高于T2,P<0.05。在T2、T3,观察组患者的HR、MAP相比,P>0.05。结论:用丁卡因对接受全麻手术的患者进行气道表面麻醉可有效地减轻其在气管插管后和拔管时的心血管应激反应。 Objective:To observe the effect of airway surface anesthesia with tetracaine on reducing cardiovascular stress response in patients undergoing general anesthesia.Methods:50 patients who underwent general anesthesia in the South Hospital of Sun Yat-sen Memorial Hospital of Sun Yat-sen University from March 2019 to August 2019 were selected as study subjects.They were randomly divided into control group and observation group.After anesthesia induction,routine endotracheal intubation was performed on patients in the control group,and airway surface anesthesia with tetracaine was performed on patients in the observation group,followed by endotracheal intubation.After surgery,heart rate(HR)and mean arterial pressure(MAP)were compared between the two groups at different times after entering the operating room.Results:after 5 min of quiet supine in the operating room(T1)and before endotracheal intubation(T2),the HR and MAP values of patients in the two groups were compared,P>0.05.Within 1 min after endotracheal intubation(T3)and 1 min after extubation(T4),HR and MAP in the observation group were both lower than those in the control group(P<0.05).In T3,HR and MAP in the control group were higher than those in T2,P<0.05.At T2 and T3,the HR and MAP values of patients in the observation group were compared to P>0.05.Conclusion:Airway surface anesthesia with tetracaine can effectively reduce the cardiovascular stress response after endotracheal intubation and extubation in patients undergoing general anesthesia.
