接受静脉留置针输液的患儿发生静脉留置针相关性感染的危险因素分析

Analysis of risk factors for intravenous indwelling needle associated infection in children receiving intravenous indwelling needle infusion

目的:分析导致接受静脉留置针输液的患儿发生静脉留置针相关性感染的危险因素。方法:将2018年1月至2019年1月期间在华西妇女儿童医院进行静脉留置针输液治疗的350例患儿作为研究对象。分析这350例患儿静脉留置针相关性感染的发生率、感染病原菌的种类及导致其发生静脉留置针相关性感染的危险因素。结果:这350例患儿静脉留置针相关性感染的发生率为15.71%。在这350例患儿的检测标本中共分离出55株病原菌,其中革兰阳性菌有29株,革兰阴性菌有18株,真菌有8株。导致本研究中患儿发生静脉留置针相关性感染的危险因素为静脉留置针的留置时间>3 d、置管位置为上肢、操作人员的年资<2年及首次置管失败。结论:导致接受静脉留置针输液的患儿发生静脉留置针相关性感染的病原菌主要为革兰阳性菌,导致其发生静脉留置针相关性感染的危险因素主要为静脉留置针的留置时间>3 d、置管位置为上肢、操作人员的年资<2年及首次置管失败。

Objective:To analyze the risk factors of intravenous indwelling needle associated infection in children receiving intravenous indwelling needle infusion.Methods:350 children who received intravenous indwelling needle infusion treatment in West China Women and Children’s Hospital from January 2018 to January 2019 were studied.The incidence,types of pathogenic bacteria and risk factors of intravenous indwelling needle associated infection in 350 children were analyzed.Results:The incidence of intravenous indwelling needle associated infection in 350 cases was 15.71%.A total of 55 strains of pathogenic bacteria were isolated from the 350 children samples,including 29 strains of gram-positive bacteria,18 strains of gram-negative bacteria and 8 strains of fungi.The risk factors for venous indwelling needle associated infection in children in this study were the indwelling time of intravenous indwelling needle>3 days,the placement of the indwelling needle on the upper limb,the operator’s seniority<2 years and the failure of the first indwelling catheter.Conclusion:lead to accept infusion of children occur vein indwelling venous indwelling needle related infections pathogen mainly gram-positive bacterium,lead to the risk factors for venous indwelling needle related infections occurred mainly for venous indwelling needle indwelling time>3 d,for the upper e xtremity,the site of insertion operator qualification<2 year s and tube failure for the first time.
