产前超声检查在胎儿先天性心脏病中的诊断价值

Diagnostic Value of Prenatal Ultrasound in Fetal Congenital Heart Disease

目的探讨产前超声检查在胎儿先天性心脏病中的诊断价值。方法选取2009年10月至2019年12月福建医科大学附属三明第一医院产科接诊的1000例胎儿疑似先天性心脏病的孕妇作为研究对象。所有孕妇入院后均接受产前超声检查,对所有孕妇的临床资料和影像学检查结果进行回顾性分析,对比产后检查结果或引产后的解剖结果,分析产前超声检查检出先天性心脏病的情况。结果1000例胎儿产后检查结果或引产后的解剖结果均证实为先天性心脏病。经产前超声检查检出951例,诊断符合率为95.1%,漏诊率为4.9%,其中四腔心切面、三血管切面对于先天性心脏病的检出率较高(均高于30%)。结论产前超声检查对于先天性心脏病的筛查具有重要的价值,且操作简单、安全可行,诊断符合率较高,能够为临床妊娠指导和后续治疗提供可靠依据。

Objective To investigate the diagnostic value of prenatal ultrasound in fetal congenital heart disease.Methods From October 2009 to December 2019,1000 pregnant women with suspected congenital heart disease in Department of Obstetrics,Sanming First Hospital Affiliated to Fujian Medical University were selected as the research objects.All pregnant women received prenatal ultrasound examination after admission,the clinical data and imaging results of all pregnant women were analyzed retrospectively.Compared with the results of postpartum examination or anatomical results after induction of labor,the detection of congenital heart disease by prenatal ultrasound was analyzed.Results One thousand fetuses confirmed as congenital heart disease by postpartum examination or anatomical results after induction of labor.Among them,951 cases were detected by prenatal ultrasound,the diagnostic coincidence rate was 95.1%,and the missed diagnosis rate was 4.9%,among them,the detection rate of congenital heart disease with four chamber view and three vessel view is higher(all higher than 30%).Conclusion Prenatal ultrasound has an important value for the screening of congenital heart disease,and the operation is simple,safe and feasible,the coincidence rate of diagnosis results is high,which can provide reliable reference for clinical pregnancy guidance and follow-up treatment.
