摘要
目的探讨老年慢性心力衰竭(CHF)患者心功能变化与血清同型半胱氨酸(Hcy)表达水平的相关性。方法选取2017年8月至2019年8月在安徽医科大学附属六安医院心血管内科收治的109例老年CHF患者为研究对象,设为试验组,另选同期40例健康体检者为对照组。检测两组患者血清Hcy和脑钠素N端前体肽(NT-proBNP)。应用超声心动图检测两组患者左室后壁厚度(LVPWT)、室间隔厚度(IVS)、左室舒张末期内径(LVEDD)、左室射血分数(LVEF)、左室质量(LVM)及二尖瓣舒张早、晚期峰值流速比值(E/A)。结果试验组患者血清Hcy、NT-proBNP以及超声检查参数(LVPWT、IVS、LVEDD、E/A和LVM)高于对照组(P<0.05),而试验组患者LVEF低于对照组(P<0.05)。Pearson相关性分析结果显示,CHF患者血清Hcy水平与NT-proBNP、LVPWT、IVS、LVEDD、E/A和LVM呈正相关(P<0.05),而与LVEF呈负相关(P<0.05)。结论血清Hcy及NT-proBNP在CHF患者中明显升高,且与CHF患者心功能有密切关系。
Objective To investigate the correlation between changes in cardiac function and serum homocysteine(Hcy)expression in elderly patients with chronic heart failure(CHF).Methods 109 cases of elderly CHF patients admitted to the department of cardiology of Liu'an Hospital Affiliated to Anhui Medical University were enrolled in the experimental group,and 40 healthy cases of people undergoing physical examination during the same period were selected into the control group.The levels of serum Hcy and N-terminal brain natriuretic propeptide(NT-proBNP)were measured in all subjects.Left ventricular posterior wall thickness(LVPWT),interventricular septal thickness(IVS),left ventricular end diastolic diameter(LVEDD)and left ventricular ejection fraction(LVEF),ratio of peak velocity of early and late diastolic mitral valve(E/A),and left ventricular mass(LVM)were measured by echocardiography.Results The levels of Hcy,NT-proBNP,and ultrasound parameters LVPWT,IVS,LVEDD,E/A,and LVM in the experimental group were significantly higher than those in the control group(P<0.05),while the level of LVEF in the experimental group was significantly lower than that in the control group(P<0.05).Pearson correlation analysis indicated that serum Hcy level in CHF patients was positively correlated with the levels of NT-proBNP,LVPWT,IVS,LVEDD,E/A,and LVM(P<0.05),while negatively correlated with the1evel of LVEF(P<0.05).Conclusion The levels of Serum Hcy and NT-proBNP were significantly increased in CHF patients,which was closely related to cardiac function in CHF patients.
