摘要
目的探讨短期应用心理支持疗法对老年膀胱癌根治术输尿管腹壁造口患者病耻感的应用效果。方法选择安徽医科大学第一附属医院2013年9月~2016年9月行老年膀胱癌根治术输尿管腹壁造口患者47例,采用随机数字表法分为对照组和干预组。对照组接受常规术后护理,干预组在对照组基础上实施心理支持护理。1个月后评价两组焦虑、抑郁情况,病耻感程度及生活质量。结果治疗后,干预组病耻感评分明显低于对照组(P<0.05);焦虑和抑郁评分明显低于对照组(P<0.05);生活质量评分明显优于对照组(P<0.05)。结论膀胱癌根治术输尿管腹壁造口的患者,术后常规护理的同时给予针对性的心理支持疗法,能够改善患者病耻感,减轻负性情绪状态,提升患者生活质量。
Objective To discuss the effect of short-term psychological intervention on stigma in older patients undergoing radical cystectomy and ureterostomy for bladder cancer.Methods 47 older patients undergoing radical cystectomy and ureterostomy for bladder cancer were selected in the First Affiliated Hospital of Anhui Medical University from September 2013 to September 2016.The patients were divided into the control group and the intervention group.The control group received routine postoperative care,the intervention group received routine postoperative care plus psychological intervention.After one month of treatment,the anxiety,depression,stigma and quality of life in both groups were evaluated.Results After treatment,the scores of stigma were lower(P<0.05),the scores of anxiety and depression were lower(P<0.05),and the scores of quality of life were higher(P<0.05)in the intervention group than those in the control group.Conclusion Combination of routine nursing combined with psychological care might reduce the stigma and the negative emotion,and improve the quality of life in patients undergoing radical cystectomy and ureterostomy for bladder cancer.
