摘要
目的应用声触诊组织量化成像(VTIQ)技术观察不同分子分型乳腺癌超声弹性特征。方法对101例术前乳腺癌患者行常规超声及VTIQ检查,观察肿块边缘带、中央区及周围组织弹性特征,测量其剪切波速度最大值(SWV max),分析不同分子分型乳腺癌的超声弹性特征。结果Luminal型弹性特征多为边缘优势型,人类表皮生长因子受体2(HER-2)过表达型主为中央优势型,三阴性多为均匀型(P<0.05)。各分子分型肿瘤边缘带SWV max Luminal A>Luminal B>HER-2过表达>三阴性(P<0.05),中央区SWV max差异无统计学意义(P>0.05)。肿瘤周围组织SWV max差异无统计学意义(P>0.05)。结论乳腺癌弹性特征及SWV max与其分子分型相关,肿瘤边缘带弹性特征尤为重要,可在一定程度上反映不同分子分型乳腺癌的生物学特点。术前VTIQ有助于制定治疗方案及评估预后。
Objective To observe elastic features of breast cancer with different molecular subtype using virtual touch tissue imaging quantification(VTIQ)technique.Methods Totally 101 patients with pathologically proven breast cancers were collected.All patients underwent preoperative routine ultrasound and VTIQ examination.The elastic features and the maximum shear wave velocity(SWV max)were obtained with VTIQ technique,and the molecular subtypes of breast cancers were recorded according to pathological findings after surgical resection.The elastic features and SWV max of different molecular subtype breast cancer were analyzed.Results The predominant elastic model of Luminal tumor was edge advantage,of human epidermal growth factor receptor-2epidermal growth factor 2(HER-2)expression type tumor was center advantage,while of triple-negative type tumor was homogeneity(P<0.05).Significant differences of SWV max in marginal region of breast cancers were found among different molecular subtypes(P<0.05),the order of SWV max of tumor margin was in turn:Luminal A type>Luminal B type>HER-2 expression type>triple-negative type.No significant difference of SWV max in central region of breast cancer was observed among different molecular subtypes(P>0.05),nor of SWV max in tumor surrounding tissue(P>0.05).Conclusion The elastic features and SWV max,especially in the marginal regions of breast cancers are related with molecular subtype,which may reflect the biological characteristics of breast cancer on certain degree.VTIQ may be helpful to treatment planning and evaluation on prognosis of breast cancer.
