王彦刚从“五脏气机升降”论治痞满经验撷英

Experience of WANG Yan-gang in the treatment of the distention and fullness from‘lifting of five zang qi movement’

痞满是中医常见病症,病情反复,病症繁多,病因病机多为表邪入里、食滞中阻、痰湿阻滞、情志失调、药物所伤、脾胃虚弱、禀赋不足等,其核心病机为中焦气机不利,脾胃升降失职;王彦刚教授从事脾胃病诊治多年,认为百病皆生于气,擅从“五脏气机升降”论治痞满,认为脾胃为气机升降之枢纽,其治疗主以调理脾胃气机升降,即升脾气、降胃气,辅以宣肺气、疏肝气、调肾气、养心气,临床疗效显著,值得推广应用,附验案一则予以佐证。

Distention and fullness is a common disease in traditional Chinese medicine,the condition is repeated and numerous.The etiology and pathogenesis are mainly pathogenic factors,food stagnation,stagnation and blockade of phlegmdamp,emotional disorders,drug injury,deficiency of spleen and stomach,insufficiency of natural endowment,etc..Its core pathogenesis is the disadvantage of qi movement of middle jiao,the failure of ascending and descending of spleen and stomach;professor WANG Yan-gang has been engaged in the diagnosis and treatment of spleen and stomach diseases for many years.He believes that all diseases are born from qi,and is good at treating the distention and fullness from the‘lifting of five zang qi movement’.It is considered that spleen and stomach are the pivot of lifting of qi movement.He believed that the main treatment was to regulate the rise and fall of spleen and stomach,that is,to‘raise the spleen qi’and‘lower the stomach qi’,supplemented by‘dispersing lung qi’‘soothing liver qi’‘regulating kidney qi’and‘nourishing heart qi’.The clinical curative effect is remarkable,and it is worth popularizing and applying,which is supported by a case report.
