产科抗磷脂抗体综合征(APS)为正虚标实之证,发病以肾虚为始动因素,分肺肾气虚、脾肾两虚、正虚血瘀、气衰血瘀四期进展。针对其正虚标实的总病机与疾病进展、证候表现特点,提出从"痹"论治的概念。二者均为正虚血瘀之证。产科APS病主要发生在血脉,以脉痹为基础演变,表现为正虚为本,在病情进展过程中正虚与瘀血的程度逐渐加重。具体原则为急则治标,缓则扶正,初益肺肾,再以健脾,久则化瘀,注重孕前孕期同治,再根据各期症状临证加减。
