连续股神经阻滞联合关节周围浸润镇痛对全膝关节置换术后关节早期功能恢复的影响

Effect of Continuous Femoral Nerve Block Combined with Periarticular Local Infiltration Analgesia on Early Recovery of Joint Function after Total Knee Arthroplasty

目的:探究连续股神经阻滞(CFNB)与关节周围浸润镇痛(PLIA)对全膝关节置换术(TKA)患者术后关节早期功能恢复的应用价值。方法:选取2016年3月-2019年3月笔者所在医院接收的行TKA患者78例为研究对象。随机分为两组,每组39例。对所有患者行PLIA治疗,研究组联合CFNB治疗,参照组联合电子静脉镇痛泵治疗。分析两组术后不同时间VAS评分、并发症发生率、关节早期功能恢复情况及生活质量。结果:研究组并发症发生率为7.68%,明显低于参照组的25.60%(P<0.05)。研究组术后不同时间VAS评分、关节早期功能恢复情况及SF-36评分均优于参照组(P<0.05)。结论:对TKA患者采用CFNB联合PLIA能促进关节早期功能恢复,减轻疼痛感,降低并发症发生率,提高患者生活质量,值得临床借鉴与推广。

Objective:To explore the application value of continuous femoral nerve block(CFNB)combined with periarticular local infiltration analgesia(PLIA)on early recovery of joint function after total knee arthroplasty(TKA).Method:A total of 78 cases with TKA admitted to our hospital from March 2016 to March 2019 were selected as the study subjects.They were randomly divided into two groups,39 cases in each group.All patients were treated with PLIA,and the study group was treated with CFNB,and the reference group was treated with electronic intravenous analgesia pump.The VAS scores at different time after operation,incidence of complications,early recovery of joint function and quality of life of the two groups were analyzed.Result:The incidence of complications in the study group was 7.68%,which was significantly lower than 25.60%of the control group(P<0.05).The VAS scores at different time after operation,early recovery of joint function and SF-36 scores in the study group were better than those of the control group(P<0.05).Conclusion:CFNB combined with PLIA for TKA patients can promote the early recovery of joint function,reduce the pain,reduce the incidence of complications and improve the quality of life of patients,which is worthy of clinical reference and promotion.
