针灸结合间歇导尿对脊髓损伤后尿潴留患者膀胱功能的影响

Effect of Acupuncture and Intermittent Catheterization on Bladder Function in Patients with Urinary Retention after Spinal Cord Injury

目的:探讨针灸结合间歇导尿对脊髓损伤后尿潴留患者膀胱功能的影响。方法:选取2017年7月-2019年7月在笔者所在医院就诊的60例脊髓损伤后尿潴留患者,按随机数表方法分为两组,各30例。对照组采用常规治疗方式,即间歇性导尿,在此基础上,观察组结合针灸治疗。对比两组的临床疗效及治疗前后的排尿情况、膀胱功能指标。结果:观察组治疗效果优于对照组,差异有统计学意义(P<0.05);治疗后,两组患者残余尿量减少,排尿时间缩短,且观察组残余尿量少于对照组,排尿时间短于对照组,差异均有统计学意义(P<0.05);治疗后,两组患者的膀胱顺应性(BC)及最大逼尿肌压力(MDP)水平均提升,且观察组高于对照组,差异均有统计学意义(P<0.05)。结论:针对脊髓损伤后尿潴留使用针灸结合间歇性导尿治疗,可有效降低患者残余尿流量,缩短排尿时间,改善膀胱功能,治疗效果显著,值得临床推广。

Objective:To investigate the effect of acupuncture and intermittent catheterization on bladder function in patients with urinary retention after spinal cord injury.Method:A total of 60 patients with urinary retention after spinal cord injury were selected in our hospital from July 2017 to July 2019,which were divided into two groups according to the random number table method,30 cases in each.The control adopted routine treatment of intermittent catheterization,on this basis,the observation group group combined with acupuncture treatment.Compared the two groups of clinical efficacy,before and after the treatment of urination,bladder function indicators.Result:The treatment effect of observation group was better than that of control group,the difference was statistically significant(P<0.05).After treatment,the amount of residual urine in the two groups decreased,and the time of urination was shortened,the amount of residual urine in the observation group was less than that in the control group,the time of urination was shorter than that in the control group,the differences were statistically significant(P<0.05).After treatment,the level of bladder compliance(BC)and maximum detrusor pressure(MDP)in the two groups increased,and the indexes in the observation group were higher than those of the control group,the differences were statistically significant(P<0.05).Conclusion:Acupuncture combined with intermittent catheterization can effectively reduce residual urine flow,speed up urination and improve bladder function after spinal cord injury.It is worthy of wide use in clinical treatment.
