床上单车运动对全膝关节置换术后患者膝关节功能恢复的影响

Effects of Cycling Motion in Bed on Knee Function Recovery in Patients Undergoing Total Knee Arthroplasty

目的:探讨床上单车运动(cycling motion in bed,CMB)对全膝关节置换术(total knee arthroplasty,TKA)后患者膝关节功能恢复的影响,为临床TKA术后有效康复训练提供可靠依据。方法:选取2017年1月-2019年1月行单侧TKA的150例患者为研究对象,随机分为对照组(75例)和观察组(75例)。对照组采用传统的持续被动训练(continuous passive motion,CPM)进行康复训练,观察组采用笔者所在科新研制的可以进行主动训练的CMB进行康复训练;分别采用术后(activities of daily living,ADL)评分对比两组患者日常生活能力,膝关节评分(hospital for special surgery knee score,HSS)评分对比两组患者膝关节功能,数字评分法(numerical rating scale,NRS)对比两组患者疼痛程度。结果:术后3个月,观察组ADL评分(82.15±10.86)分,与对照组的(65.32±8.02)分相比明显较高(P<0.05);观察组HSS评分(79.24±9.18)分,与对照组的(62.13±8.62)分相比明显较高(P<0.05);观察组NRS评分(2.28±1.23)分,与对照组的(3.46±1.28)分相比明显较低(P<0.05)。结论:CMB在单侧TKA术后关节功能的恢复中,具有提高患者日常生活能力,改善患者关节功能并降低患者疼痛感的作用,值得在临床TKA术后患者中进一步推广应用。

Objective:To investigate the effect of cycling motion in bed(CMB)on knee function recovery after total knee arthroplasty(TKA)in order to provide a reliable basis for effective rehabilitation training after clinical TKA.Method:A total of 150 patients who underwent unilateral TKA from January 2017 to January 2019 were selected as the study objects,and were randomly divided into a control group(75 cases)and an observation group(75 cases).The control group used traditional continuous passive training(CPM)for rehabilitation training.The observation group used the newly developed CMB that can be actively trained for rehabilitation training.The activities of daily living(ADL)scores were used to compare the two groups of patients’daily life ability and knee joint score[hospital for special surgery knee score(HSS)]was used to compare knee function between the two groups,and a numerical rating scale(NRS)was used to compare pain between the two groups.Result:Three months after the operation,the ADL score of the observation group was(82.15±10.86)points,significantly higher than(65.32±8.02)points of the control group(P<0.05);the HSS score of the observation group was(79.24±9.18)points,which was significantly higher than(62.13±8.62)points of the control group(P<0.05);the NRS score of the observation group was(2.28±1.23)points,which was significantly lower than(3.46±1.28)points of the control group(P<0.05).Conclusion:CMB has the effect of improving the daily living ability,improving the joint function and reducing the patients’pain in the recovery of joint function after unilateral TKA.It is worthy of further popularization and application in clinical TKA patients.
