胃底静脉组织胶注射治疗食管胃底静脉曲张破裂出血的临床效果

Clinical Effect of Gastric Fundus Tissue Histoacryl Injection on Esophagus-stomach Fundus Varices Hemorrhage

目的:探讨胃底静脉组织胶注射治疗食管胃底静脉曲张破裂出血的临床效果。方法:将2017年3月-2019年3月笔者所在医院收治的食管胃底静脉曲张破裂出血患者70例依据随机数表法分为对照组和观察组,每组35例。对照组使用注射硬化剂的方式治疗,观察组接受内镜下组织胶注射治疗。治疗后对比两组的止血效果,包括止血成功率、再次出血率、静脉曲张消失率及复发率,同时记录并发症发生情况。结果:观察组止血成功率91.43%(32/35)、静脉曲张消失率91.43%(32/35),与对照组的77.14%(27/35)、71.43%(25/35)相比较高,且观察组的再出血率及复发率均低于对照组,差异有统计学意义(P<0.05);对照组并发症总发生率22.86%(8/35),高于观察组的2.86%(1/35),差异有统计学意义(P<0.05)。结论:针对食管胃底静脉曲张破裂出血患者使用组织胶注射治疗,可有效提高止血成功率及静脉曲张消失率,降低再出血率及静脉曲张复发率,同时并发症发生率较低,不仅疗效理想且安全性较高,值得临床推广。

Objective:To explore the clinical effect of gastric fundus tissue histoacryl injection on the treatment of esophagus-stomach fundus varices hemorrhage.Method:According to the random number table method,70 patients with esophagus-stomach fundus varices hemorrhage treated in the hospital from March 2017 to March 2019 were divided into the control group and the observation group,35 cases in each group.The control group was treated by hardener injection,and the observation group received histoacryl injection.After treatment,the hemostatic effect of the two groups were compared,including the success rate of hemostasis,the rate of rebleeding,the disappearance rate of varicose veins,and the rate of recurrence,and the occurrence of complications.Result:The success rate of hemostasis in the observation group was 91.43%(32/35),and the disappearance rate of varicose veins was 91.43%(32/35),which was higher than the control group’s 77.14%(27/35)and 71.43%(25/35).And the rebleeding rate and recurrence rate of the observation group were lower than the control group,the differences were statistically significant(P<0.05);the total incidence of complications in the control group was 22.86%(8/35),higher than 2.86%of the observation group(1/35),the difference was statistically significant(P<0.05).Conclusion:The use of histoacryl injection for patients with esophagus-stomach fundus varices hemorrhage can effectively improve the success rate of hemostasis and the disappearance rate of varicose veins,reduce the rebleeding rate and the recurrence rate of varicose veins,and at the same time,the incidence of complications is low.It not only has ideal therapeutic effect and high safety,but also worthy of clinical promotion.
