关节镜下普通1.2克氏针塑形成倒鱼钩样治疗胫骨髁间嵴骨折的临床应用价值

Clinical Application Value of Ordinary 1.2 Kirschner Wire Plastic Forming Inverted Fishhook Shaped under Arthroscopy in the Treatment of Tibial Intercondylar Eminence Fracture

目的:探讨胫骨髁间嵴骨折采取关节镜下普通1.2克氏针塑形成倒鱼钩样治疗的临床应用效果及远期价值。方法:选取2017年2月-2019年7月深圳市坪山区人民医院骨科收治的40例胫骨髁间嵴骨折患者,根据入院先后顺序分为观察组和对照组,各20例。对照组采用高强线"8"字缠绕固定方法,观察组采用普通1.2克氏针塑形成倒鱼钩样关节镜下微创治疗,评价两组治疗指标、术后膝关节功能评分、并发症发生率及生活能力评分。结果:观察组手术时间、住院时间均短于对照组,差异均有统计学意义(P<0.05);两组骨折愈合时间比较,差异无统计学意义(P>0.05);两组术后6个月HSS膝关节功能评分均较术前显著提高,且观察组术后6个月HSS膝关节功能评分高于对照组,差异均有统计学意义(P<0.05);观察组术后并发症发生率低于对照组,差异有统计学意义(P<0.05);两组术后生活能力评分均较术前明显下降,且观察组术后生活能力评分低于对照组,差异均有统计学意义(P<0.05)。结论:胫骨髁间嵴骨折应用关节镜下普通1.2克氏针塑形成倒鱼钩样治疗方法更具优势,安全性高,更有利于提高患者生活能力,有较高的推广与应用价值。

Objective:To investigate the clinical effect and long-term value of ordinary 1.2 Kirschner wire plastic forming inverted fishhook shaped under arthroscopy in the treatment of tibial intercondylar eminence fracture.Method:A total of 40 patients with tibial intercondylar eminence fracture who were admitted to the orthopaedic department of Pingshan People’s Hospital from February 2017 to July 2019 were selected,and they were divided into the observation group and the control group according to the order of admission,20 cases in each group.The control group was treated with high strength line "8" winding and fixation method,and the observation group was treated with ordinary 1.2 Kirschner wire plastic forming inverted fishhook shaped under arthroscopy.The treatment indicators,postoperative knee joint functional scores,incidence of complications and living ability scores of the two groups were evaluated.Result:The operation time and hospitalization time in the observation group were shorter than those of the control group,and the differences were statistically significant (P<0.05).The fracture healing time of the two groups was compared,and the difference was not statistically significant (P>0.05).And 6 months after operation,the HSS knee joint functional scores in both groups were significantly improved compared with those before operation,and the HSS knee joint functional score in the observation group was higher than that of the control group,and the differences were statistically significant (P<0.05).The incidence of postoperative complications in the observation group was lower than that of the control group,and the difference was statistically significant (P<0.05).The postoperative living ability scores of the two groups were significantly lower than those before operation,and the postoperative living ability scores of the observation group was lower than that of the control group,and the differences were statistically significant (P<0.05).Conclusion:The treatment of tibial intercondylar eminence fracture with ordinary 1.2 Kirschner wire plastic forming inverted fishhook shaped under arthroscopy has more advantages,higher safety,more conducive to improving the living ability of patients,and has higher promotion and application value.
