液体复苏结合参附注射液治疗严重创伤失血性休克的临床效果

Clinical Effect of Fluid Resuscitation Combined with Shenfu Injection in the Treatment of Severe Traumatic Hemorrhagic Shock

目的:研究液体复苏结合参附注射液治疗严重创伤失血性休克患者的临床效果。方法:选取2017年2月-2018年2月于笔者所在医院就诊的严重创伤失血性休克患者86例,按照随机数字表法将其分为对照组(43例)和研究组(43例)。对照组行常规液体复苏急救方案,研究组在对照组基础上行参附注射液的急救方案。比较两组休克恢复状态、炎症指标、凝血指标、并发症发生率。结果:研究组休克指数显著低于对照组,乳酸清除率显著高于对照组,差异均有统计学意义(P<0.05)。研究组IL-6、TNF-α水平均显著低于对照组,差异有统计学意义(P<0.05)。研究组TT、PT、APTT均显著低于对照组,差异有统计学意义(P<0.05)。研究组并发症发生率为11.63%,显著低于对照组的46.51%,差异有统计学意义(P<0.05)。结论:液体复苏结合参附注射液治疗严重创伤失血性休克可显著缓解患者休克状态,降低炎症水平,改善凝血功能,降低并发症发生率,可临床推广。

Objective:To study the clinical effect of fluid resuscitation combined with Shenfu Injection in the treatment of patients with severe traumatic hemorrhagic shock.Method:A total of 86 patients with severe traumatic hemorrhagic shock who were treated in our hospital from February 2017 to February 2018 were selected and divided into the control group (43 cases) and the study group (43 cases) according to the random number table method.The control group received routine fluid resuscitation emergency plan,and the study group received Shenfu Injection emergency plan on the basis of the control group.The recovery state of shock,inflammation index,coagulation index and complication rate were compared between the two groups.Result:The shock index of the study group was significantly lower than that of the control group,the lactate clearance rate was significantly higher than that of the control group,the differences were statistically significant (P<0.05).The levels of IL-6 and TNF-α in the study group were significantly lower than those of the control group,and the difference was statistically significant (P<0.05).The TT,PT and APTT in the study group were significantly lower than those in the control group,and the differences were statistically significant (P<0.05).The incidence of complications in the study group was 11.63%,which was significantly lower than 46.51% in the control group,and the difference was statistically significant (P<0.05).Conclusion:The combination of fluid resuscitation combined with Shenfu Injection in the treatment of severe traumatic hemorrhagic shock can significantly alleviate the shock state of patients,reduce the level of inflammation,improve the coagulation function,reduce the incidence of complications,and can be widely used in clinical practice.
